- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Social Security Administration has a requirement for one (1) year of maintenance beginning on November 16, 2004 on the following government-owned equipment: 213 Intermec Janus J2020 Scanners, 192 Intermec Janus JD2020C Communications Docks and four (4) Intermec Janus J2010 collectors; Solicitation # SSA-RFQ-05-1089. This is a combined synopsis/solicitation for commercial items (as defined by FAR 2.101) prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Offers are being requested; however, a written request for offers will not be issued. This acquisition is subject to FAR Subpart 13.5, regarding the application of simplified acquisition procedures. This is a small business set-aside. FAR 52.219-6 applies to this RFQ. Quotations submitted from other than small business sources will be rejected. The small business size standard is 750 employees. The North America Industry Classification System (NAICS) code for this acquisition is 811212. REQUIREMENTS/ SPECIFICATIONS: 1) On-call support is needed Monday – Friday, 8:30am – 5pm. Vendor will provide an 800 number with the quote. 2) Equipment locations are as follows: Baltimore, MD; Jamaica, NY; Philadelphia, PA; Birmingham, AL; Chicago, IL; Kansas City, MO; and Houston, TX. TERMS AND CONDITIONS: Far Part 52.212-4, Contract Terms and Conditions-Commercial Items (Oct 2003) is applicable to this order. The following provisions at FAR Part 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items March 04 apply to this acquisition: (b)(5), (b)(14), (b)(16), (b)(17), (b)(18), (b)(19), (b)(20), (b)(31). FAR Part 52.212-1, Instructions to Offerors-Commercial, applies to this order. EVALUATION: The provisions at FAR Part 52.212-2, Evaluation--Commercial Items are not applicable to this acquisition. The government will award an order resulting from this request for quotation to the vendor whose quote represents best value to the Government, price and past performance/experience considered. Vendors must provide documentation supporting their ability to perform this maintenance. REPRESENTATIONS AND CERTIFICATIONS: Vendors must include a completed copy of the provisions at FAR Part 52.212-3 updated through FAC 2001-24, Offeror Representations and Certifications -- Commercial Items, with their quote.
