- Description
- The VA Medical Center, Bay Pines, Florida has a requirement to provide veterans within the service area of Sarasota, Florida with audiologic services. Services will be performed at contractor facility by a VA clinician and support staff. Contractor facility must be located in the Sarasota, Florida area. Contractor facility must be JCAHO compliant. It is the Department of Veterans Affairs (VA) intention to solicit proposals in accordance with all terms, conditions, provisions, specifications, and schedules of this solicitation and award a contract on a competitive basis under the authority of Public Law 104-262 and 38 U.S.C. 8153. The solicitation will be conducted in accordance with FAR 13.5 test program with the procurement conducted using FAR Parts 12 and 15 in accordance with IL 90-99-4. The VA anticipates the award of a single contract of a firm, fixed-price indefinite delivery/indefinite quantity basis from this solicitation. The contract period will be one year from the date of award, with the Government having the option to extend the contract for four (4) additional one-year periods. No hard copy solicitations will be issued. Faxed or telephone requests for solicitation will not be honored. Contractors may obtain a copy of the solicitation and any attachment at the following URL: http://bos.oamm.va.gov. Contractors are advised that they are responsible for obtaining any amendments. All offers must be returned to the address in block 9 of Standard Form 1449. NAICS (North American Industry Classification System) code 621340 applies.
