- This is Amendment 0004 for combined synopsis/solicitation H92222-04-R-0037. The purpose of this Amendment is to correct the following: 1. Delete Schedule B in its entirety and replace with the following Schedule B: *** BEGIN CONTRACT LINE ITEMS *** Schedule B CLIN 0001 - Develop technical surveillance curriculum – Firm Fixed Price (FFP) CLIN 0002 - Perform instruction IAW the developed curriculum, Initial Course (Four (4) Weeks)Firm Fixed Price (FFP);SLIN 0002AA (Base Year); SLIN 0002AB (Option Year); SLIN 0002AC (Option Year 2; SLIN 0002AD (Option Year 3); SLIN 0002AE (Option Year 4) CLIN 0003 – Perform instruction IAW the developed curriculum Refresher Course (Two (2) Weeks)Firm Fixed Price (FFP) SLIN 0003AA (Base Year); SLIN 0003AB (Option Year 1) SLIN 0003AC (Option Year 2) SLIN 0003AD (Option Year 3) SLIN 0003AE (Option Year 4) CLIN 0004 - Contract Data Requirements List, DD1423 NSP SLIN 0004AA (Base Year)(Not Separately Priced (NSP)). SLIN 0004AE (Option Year 4)(Not Separately Priced (NSP)). 2 Correct 52.212-2 Addendum Evaluation paragraph – Commercial Items (a) Basis for Award, (2) Evaluation Approach, (C) Area III: Price and replace with the following: (C) AREA III: PRICE. 1) Completeness and Balanced Pricing. The proposal will be evaluated to ensure that ALL CLINS for which the offeror is submitting prices are completed. All proposals will be evaluated for unbalanced pricing. Offerors are cautioned that unbalanced unit prices may be grounds for exclusion of a proposal from consideration for award. 2) Price reasonableness shall be determined through competitive commercial market prices and IAW the guidelines set forth in FAR 15.404-1. 3) Price will be evaluated for completeness, reasonableness and a total evaluated price (TEP) for the entire length of the contract. 4) The TEP will consist of the summation of the following: · the extended price of CLIN 0001; · the unit price of CLIN 0002 multiplied by four (4) classes for the base year and all options twenty (20) total classes and, · the unit price of CLIN 0003 multiplied by five (5) classes for the base year and all options twenty-five (25) classes total. This amendment extends the date for receipt of proposals to 14 Oct 04 @ 1600.
