Description

The Contractor shall furnish Courier Service with a company vehicle and uniformed driver to transport Blood and Blood products (in approved container- provided by the VA or Red Cross.) from The American Red Cross, 100 Mack Ave., Detroit, Michigan 48232-5351 to the VA Ann Arbor Healthcare System, 2215 Fuller Road, Blood Bank (Building #1, 3rd Floor, Room #E330), Ann Arbor, Michigan 48105 for the period of November 1, 2004 through September 30, 2004, in strict accordance with all specification, schedules, terms, conditions, and provisions of this solicitation and any resulting contract. For the purpose of this procurement, a trip is defined as follows: Pick up made from The American Red Cross with Delivery made to the VA Ann Arbor Healthcare System, 2215 Fuller Road, Blood Bank (Building #1, 3rd Floor, Room#E330) Attn: Blood Bank Supervisor, Designee (list of authorized personnel will be provided after award) or COTR exclusively. Pick-up?s/Delivery shall be made in accordance with the time constraints as noted in section VI. Quality Assurance. In extremely rare cases of Blood Shortages trip may require alternate pick-up locations to be determined by the American Red Cross. The Contractor shall ensure the confidentiality of all patient information being transported and shall be held liable in the event of breach of confidentiality. The Contractor shall comply with the provisions of the Federal Privacy Act of 1974 (Public Law 93-579)
