- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Naval Special Warfare Development Group, Virginia Beach, Virginia has a potential requirement to procure the following: Clin 0001 Custom Orthotics (OA II Sport), Quantity 14 each; Clin 0002 Custom Orthotics (Tenderfoot Soft), Quantity 5 each; Clin 0003 Custom Orthotic (Tenderfoot Firm), Quantity 100 each; Clin 0004 Custom Orthotic (Tenderfoot Medium), Quantity 50 each; Clin 0005 Custom Orthotic (Sport), Quantity 10 each; Clin 0006 Custom Orthotic (Medial Wedge), Quantity 5 each; Clin 0007 Custom Orthotic (Action Arch), Quantity 25 each; Clin 0008 Custom Orthotic (Tenderfoot Soft), Quantity 5 each; Clin 0009 Custom Orthotic (Carbo Lite Plate), Quantity 5 each; Clin 0010 Custom Orthotic (Graphite Athletic), Quantity 5 each; Clin 0011 Custom Orthotic (Golfer), Quantity 5 each. This Request for Quote (RFQ) H92244-04-Q-0089 is anticipated to result in a Firm Fixed Price (FFP), Indefinite Delivery, Indefinite Quantity (IDIQ) contract. Funding shall be provided on each individual delivery order issued. Names and Sizes of custom orthotics will vary on each delivery order depending upon need of end user. Submit written offers; oral orders will not be accepted. This combined synopsis/solicitation is issued on an Unrestricted basis with full and open competition. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-24 dated 19 July 2004. North American Industrial Classification Code (NAICS) is 423450, size standard 100 employees, applies to this procurement. Deliver FOB: Destination, NSWDG, 1636 Regulus Avenue, Virginia Beach, VA 23461-2299. The DPAS rating for this procurement is DO. The following provisions and clauses apply to this acquisition: The provision at FAR 52.212-1 Instructions to Offerors Commercial Items (Oct 2000) applies to this acquisition. Proposals shall include the vendor?s Cage Code, Dun and Bradstreet Number (DUN) and Tax Identification Number (TIN).
