Description

Provide all supplies, materials, labor, transportation, travel and associated costs to provide and install one (1) Topcon TRC-50EX Fluorescein Retinal Camera, one (1) Topcon AIT-15 Electric Instrument Table, one (1) Hewlett Packard 2230ix Business Inkjet Printer, one (1) Topcon 1280 X 1024 FA/Color Capture System, and one (1) IMAGEnet Worklist Modality Module, or equal at the VA Outpatient Clinic located in Ft. Myers, Florida. Contractor to provide two (2) days of training to the end user. All work is to be performed in accordance with the guidelines established by Federal, State and local ordinances, with industry standards/manufacturers? guidelines, and with all terms, conditions provisions, schedules and specifications of this solicitation. Hard copy solicitations will not be made available. Contractors may obtain a copy of the solicitation and any attachments at the following URL: http://www.bos.oamm.va.gov. Contractors are advised that they are responsible for obtaining any amendments. All quotes must be returned to the address in block 9 of Standard Form 1449. North American Industry Classification System (NAICS) code 421460 applies. This is a 100% total Service Disabled Veteran Owned Small Business set-aside. This solicitation is being issued as a Commercial Item Acquisition in accordance with FAR Part 12, and will be issued on or about September 27, 2004 with quotes due on or about September 30, 2004
