Description

This modification is being issued to replace the Performance Work Statement and Quality Assurance Surveillance Plan. Both documents contained new dates of 29 Jul 04 and will be uploaded today, 29 July 04. Travis AFB is seeking to acquire commerical services IAW FAR part 12 and 13.5 to perform transcription services for the radiology department at travis AFB. Contractor will be expected to furnish all labor, tools, materials, and transportation to provide this service. Complete details of services and qualifications will be provided in the performance work statement (PWS). The performance period is a base year and four one year options, from date of award through 30 Sep 2009 if all options are exercised. This is 100% small business setaside under NAICS Code is 561410. The related Small Business Size Standard is $6 million. This will be a Request for Proposal. Award will be made on based upon Best Value to the government in accordance with FAR 15 using Price Performance Trade-Off (PPTO).. The government reserves the right to award a contract to other than the low proposed offer. If you are the successful offeror, the government intends to award a 15 days purchase order to phase in your firm prior to contract start date to obtain base identification and provide required systems training for the period 15 Sep through 30 Sep 04. The solicitation package will be posted in FedBizOpps, which can be accessed at http://www.eps.gov, on or about 16 Jul 04. THESE DATES ARE ONLY ESTIMATES AND ARE SUBJECT TO CHANGE WITHOUT NOTICE. The specific closing date will be stated in the solicitation package. The government will not provide paper copies of the solicitation package. Facsimile, telephone, written, or e-mail requests for the solicitation package will not be honored.
