Description
Combined Synopsis/Solicitation: PETERSON AFB, BLDG 1 ANNUAL PLANT MAINTENANCE. (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the formation in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for proposal (RFP). Submit written quotes (oral quotes will not be accepted) on RFP solicitation number FA2517-04-T-6012. (III) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. (IV) This solicitation is for a small business set-aside and the associated NAICS code is 453110 with a small business size of 500 employees. (V) This requirement includes weekly inspection and maintenance of all plants in Bldg 1 to include: proper moisture level, clean and trim plants, remove debris from soil, add soil/mulch to soil surface, rotate plants to predominant light sources, provide proper levels of nutrients, inspect and treat for disease or insect infestation (IAW federal and local regulations), clean all portable planters and pots as required and requested. Also, guarantee all plants and replace with equal plants at no cost when plants: expire, become diseased, reach maximum growth in pot or determined unhealthy by customer. Plant List : Ficus Tree, Marginata Branch, Bamboo Palm, Fish Tail Palm, Amate Shcefflera, Drac. Marginata, Drac. Mass. Cane, Kentia, Norfolk Island Pine, Janet Craig, Drac. Janet Craig Cane, Mass. Cane, Warneckii, Warneckii Tips, Spath, Neanthe Bella Palm, Pothos, Pothos ? HB, Philo-Selloum, Dwarf Shefflera, Croton, Var. Arboricola, Rhapis Palm, Reflexa, Sanseveria, Majesty Palm, Aglaonema, Spath Sensation, Aglo-Hybrid, Corn Tips, Ivy-Hanging Baskets, Pole Pothos, Algerian, Drac. Marginata, Aspidistra. There are a total of 1,000 plants. (VI) The period of performance is from 1 Oct 2004 to 30 Sept 2005 (Basic Contract year plus four Option years). Place of performance is 150 Vandenberg Street at Peterson AFB, CO.80914. (VII) All firms or individuals responding must be registered with the Central Contractor Registration (CCR) at http://www.ccr.gov/. (VIII) The following provisions and clauses apply to this acquisition: FAR 52.212-1, Instructions to Offerors ? Commercial Items (Jan 2004), applies to this acquisition. In addition to written quotes for basic and all option years, offerors are instructed to provide a capability statement describing qualifications to perform the requirement as listed above. (IX) FAR 52.212-2, Evaluation -- Commercial Items (Jan 1999), Paragraph (a) of this provision is completed as follows: (I) price and (II) capability statement (pass/fail assessment) shall be used to evaluate offers. Only those offerors receiving a ?pass? assessment will be considered for award. (X) The offeror must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (May 2004). A copy of this provision is located at http://farsite.hill.af.mil/. (XI) The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (Oct 2003). (XII) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (Jun 2004). Additional FAR 52.212-5 clauses cited in the clause applicable to this acquisition are (b) (5)(i) (14) (16) (17) (18) (19) (20) and (30), (c) (1), (2), and (3). (XIII) The following clauses are also applicable to this acquisition: 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items, 252.204-7004 Required Central Contractor Registration, and 252.232-7003 Electronic Submission of Payment Requests. (XIV) The Government intends to award a single Firm Fixed Priced contract resulting from this solicitation.
