Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A quote is being requested and a written solicitation will not be issued. Solicitation number F73MED42373100 is hereby issued as a Request for Quote. FUNDS ARE NOT CURRENTLY AVAILABLE FOR THIS REQUIREMENT, HOWEVER IT IS INCLUDED IN THE FY 05 BUDGET. AWARD IS CONTINGENT UPON FUNDS BECOMING AVAILABLE ON OR AFTER 1 OCTOBER 2004. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-15. This acquisition is a 100% set aside for small business and is under North American Industry Classification System code 334517, with a small business size standard of 500 employees. PRICE SCHEDULE: Offers should be submitted for CLIN 0001: Quantity 2880 ROUTINE (2 day turnaround time) READINGS @ ______ = $_______. CLIN 0002: Quantity 320 STAT ( 1 Day turnaround) READINGS @ ______ = $______. Quantities are not to exceed amount. The contractor will invoice on a monthly basis for the work performed. DESCRIPTION: 1.1 The Radiology Department of the 97th Medical Group will be the only point of contact for arranging interpretation of developed x-ray film for the 97th Medical Group. 1.2 The contractor shall provide evidence that employees reviewing the devloped x-ray film are board certified, or board eligible, by the American Board of Radiology and shall possess an unrestricted medical license to practice radiology from a U.S. jurisdiction. 1.3 The contractor shall submit their detailed quality assurance program to the government prior to the start of services. PERIOD OF PERFORMANCE: 1 Oct 04 through 30 Sep 05. STATEMENT OF WORK: See attached document. CONTRACTOR MUST HAVE A NATIONAL AGENCY CHECK AND INDUSTRIAL CLEARANCE. The following clauses and provisions are incorporated and are to remain in full force in any resultant purchase order: FAR Clause 52.212-1, Instructions to Offerors- Commercial Items (Jan 2004). It is incorporated by reference and applies to this acquisition. It is amended to read: Submit signed and dated offer to 97 CONS/LGCA, 303 J Ave Bldg 302, Altus AFB OK 73523-5132 at or before 1600 (4:00 P.M) CST, 21st September 2004. Submit dated quotations on company letterhead or quotation form. Quotations may be faxed to (580) 481-5138. Each offeror shall include a completed copy of the provisions at FAR clause 52.212-3, Offeror Representations and Certifications?Commercial Items (May 2004) with the quotation.
