- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation FA4877-04-Q-0510 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular _01-24__. (iv) This procurement is being issued as a 100% SMALL BUSINESS set-aside under NAICS code _517910_______ and small business size standard _12 M. (v) The following line items apply to the Aircrew Life Support Equipment Records Tracking System (ALERTS) request: 1.) Symbol PDT8146 (including Cradle/Spare Battery), Qty: Three (3) 2.) Symbol P302FZY Keyboard Scanner, Qty: Three (3) 3.) Zebra Z4000 300 DPI Printer model 4000-201, Qty: Two (2) 4.) Small Label 1.5 x .5 part #65590, Qty: Two (2) 5.) Large Label 3 x 1 part #65677, Qty: Two (2) 6.) Ribbon Large 4100BK08045, Qty Two (2), 7.) Tablet Computer-Fijitsu Stylistic 3500 model FMW4503TS128A03 with Mini Dock, Qty: Two (2), 8.) Portable Printer-HP 340 or Equivalent, Qty: Two (2) (vi) This requirement is for an Aircrew Life Support Equipment Records Tracking System (ALERTS) 9. Delivery and Installation of the ALERTS System (vii) (ALERTS) is 30 day upon contractor receipt of order, delivery location is 357 FS/DOL, 3740 S. 1st Street, Blding 5247, Davis-Monthan AFB 85707, FOB destination.(viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition.(ix) 52.212-2, Evaluation -- Commercial Items does not apply. Quotes will be evaluated on price. (x) N/A Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer.(xi) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
