- This combined synopsis/solicitation is for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The Solicitation Number is HE1254-04-R-0064 and is issued as a request for proposal (RFP). This action will be processed using simplified acquisition procedures and incorporates provision and clauses that are in effect through Federal Acquisition Circular Fac 2001-24 and DCN 20040625 Edition. The Department of Defense Education Activity (DoDEA) is a federal government agency, tax exempt, an education activity and qualifies for academic/educational pricing. Proposals shall include offeror's best and final offer. FSC Code: 7610, Book & Pamphlets. NAICS Code: 511130, Book Publishers. Size Standard: 500 Employees. GENERAL REQUIREMENTS: DoDEA requires educational materials and services for a music curriculum program covering general, vocal, and instrumental music courses in grades Pre-Kindergarten-12. The program consists of three distinct areas; Pre-Kindergarten Music, General Music (Grades K-8), and Vocal and Instrumental Music (Grades 7-12). THE FOLLOWING ITEMS SHALL BE SUBMITTED IN WRITING Proposals for this RFP must be complete, signed, dated, and due in this office at the Department of Defense Education Activity (DoDEA), Procurement Division, 4th Floor, 4040 N. Fairfax Drive, Arlington, Virginia 22203-1635, no later than 3:00 p.m., EST, Tuesday, November 16, 2004. Proposals shall be marked with the HE1254-04-R-0064 and addressed to Mr. Carl Toepel. All documents and materials must be submitted in accordance with the attached RFP. Central Contractor Registration (CCR) Database (www.ccr2000.com) and Electronic Funds Transfer (EFT) are mandatory. Proposals shall include CAGE code, DUNS number, Tax ID number, email address, business size (large/small), and web site address, if applicable.
