- Description (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. (ii) Solicitation FA8477-04-Q0675 is issued as a Request for Quote (RFQ) using simplified acquisition procedures under the test program for certain commercial items found at FAR 13.5. (iii) The solicitation document incorporates provisions and clauses that are in effect through Federal Acquisition Circular (FAC) 2001-14. It is the vendor?s responsibility to be familiar with all applicable clauses and provisions. (iv) This procurement is a 100% Small Business Set Aside. The North American Industry Classification System -- (NAICS) code is 326191 with a size standard of 500 employees. (v) Description: CLIN 0001: ninteen (19) each Standard Chemical Toilets (tan) (vi) Requirement: Must be Polyportables, OR EQUAL. CLIN 0002: two (2) each special needs chemical toilets (blue), Must be Polyportables, OR EQUAL. CLIN 0003: one (1) lot, any applicable frieght charges. Offerors shall include the manufacturer or brand name of the product and a statement of equivalence, which clearly documents that the offered product meets or exceeds the performance of the above item in all respects. Price quotes shall include all applicable sales taxes and shipping charges. (vii) Delivery: Date of delivery shall be no later than 45 Days After Receipt of Award. Place of delivery and acceptance will be CE Supply Store/GOCESS, Bldg 5320, Davis-Monthan AFB AZ 85707. The vendor shall deliver CLIN 0001/0002/0003 FOB Destination. (viii) FAR 52.212-1, Instructions to Offerors-Commercial Items (OCT 2000) is hereby incorporated by reference. (ix) FAR 52.212-2, Evaluation Commercial Items (JAN 1999) is hereby incorporated by reference. The Government will award a purchase order resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation will be most advantageous to the Government. Price and price related factors will be considered. (x) Offerors are reminded to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications _ Commercial Items (JUN 2003). (xi) FAR 52.212-4, Contract Terms and Conditions _ Commercial Items (FEB 2002), is hereby incorporated by reference. (xii) FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders _ Commercial Items (JUN 2003), is hereby incorporated by reference.
