Description

This is a Solicitation for Commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12 (Acquisition of Commercial Items) and with FAR Subpart 13 (Simplified Acquisition Procedures). This is a small business set aside. Small business set-aside is 500 employees. Request for Quotation (RFQ) 673-177-04 is assigned and shall be reference on any request for solicitation. North American Industry Classification System is 333921. The requirement is brand name or equal in accordance with VAAR Clause 852.211-77. Delivery to be made to James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, Florida 33612. Provide By Price the Following: Five (5), 04-05 Chevy Express 3500 Vans. Conversions of GSA Vans to Wheelchair Litter Vans. Tag Numbers 43-1377A, 43-1399a, 43-1400A, 431351A, 43-1376A. Remove existing rear bench seating floor matting & seat brackets. Provide/Install Plywood Sub-Flooring & Non-Skid Vinyl Flooring from rear of front seats back. Provide/install recessed tie-downtracks & (3) sets of tie-down belts. Provide/Install 24_ Raised Roof & Roof Support System, finished to match van. Provide/Install raised side cargo doors, finished to match van. Prove/Install Braun Millenium L917 ADA Wheelchair Lift. Remove existing side cargo door panels & replace w/mat material. Provide/install jump seat over passenger rear wheelwell. Provide/Install med/02 Box. Install customer furnished stretcher tie-down equipment. Provide/Install interior modifications for raised rood/raised door area. Install customer furnished cargo net & fire extinguisher.
