Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is NTSBQ040043 and the solicitation is issued as request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. This is an unrestricted procurement. The associated NAICS code is 423490 and the small business size standard is 500 employees. The NTSB has a requirement for Bloodborne Pathogen Kits as described below. The following shall be packed together in a sturdy plastic bag, sealed with the contents including suit size and quantities labeled on the front of each kit: DuPont/Kappler ProShield "2" NexGen coverall with elastic back, zipper front, attached hood, skid resistant attached boots, elastic wrist and face opening and serged seams. Color: white Qty: 1 Pair of "High Risk", 14 mil, 12" cuff latex gloves. Qty: 2 Pair of Nitrile 11" long, 6 mil, powder-free, puncture resistant, non-latex gloves. Qty: 1 Pair of heavy-duty canvas/safety cuff leather palm gloves. Qty: 1 Pair of fog-free indirect vented goggles (clear). Qty: 1 Pair of latex boot covers with non-slip sole. Qty: 1 N95 Particulate respirator with exhale valve. Qty: 2 Head sweatband. Qty: 1 Antimicrobial hand wipes. Qty: 4 Surface disinfectant wipes. Qty: 2 Red biohazard waste bags. Qty: 2 Instruction Sheet/Incident Report form. Qty: 1 Small bottle of Purrell hand sanitizer. Qty: 1 Ten kits of the same size shall be packed in one case. Each case shall contain one roll of Duct Tape, 2"x 60 yards. The sizes to be delivered to each location will be specified in the resultant contract. Also provide per case prices for option year one and option year two for each size kit. The option years will be structured as a requirements contract with the estimated annual quantity of 91 cases. However the NTSB may order fewer than or more than 91 cases as determined by our actual requirements in each of the option years. Option exercise clauses and ordering provisions will be included in the resultant contract. If the per case price will vary depending on the quantity ordered include than information in you quotation. Offerors must quote on the requirements as stated. If awarded the contract, failure to include the required items in the kits will be grounds for termination for default. Inspection and acceptance will be at destination. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Offeror is required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer.
