(note that quantities represent estimated labor hours)MILSTRIP: N0018304RQMZ001PURCHASE REQUEST NUMBER: N0018304RQMZ001 NET AMT FOB: Destination ITEM NO SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0003 133 Hours Onsite Technical ServiceFFPfor the Installation & Configuration of a Softmed: Orthoemr Data Web Query System (note that quantities represent estimated labor hours)MILSTRIP: N0002303RCEBZW9PURCHASE REQUEST NUMBER: N0002303RCEBZW9 NET AMT FOB: Destination Section C - Descriptions and Specifications CLAUSES INCORPORATED BY FULL TEXT THE MINIMUM REQUIREMENTS OF THIS SOLICITATION ARE LISTED BELOW: ?h Vendor to provide installation / integration of (1) Data Webb Query System (2) Hospital Admission Templat and (3) Data Webb Query Systems to the Orthopedic Clinic software systems for the Naval Medical Center Portsmouth (NMCP). Required product is currently manufactured by Softmed. Vendor to supply quote for Softmed Software or a copatable ??off the shelf?? software package. In the event that Softmed software is to besubsituted. Supplierd software must be fully compatiable with current Softmed system. No custom software accepted. In the event that Softmed products are not proposed the Government reserves the right to request techanical data and shall be the final auhotity in determining ??equal or better??. o Upon final implementation vendor to ensure that supplied software is of the latest version available. o Vendors price to be inclusive of all cost, including all license, upgrades, parts, labor and transportation (including Shipping, employee travel and per-diem) o All required initial training and set-up support including Project Management assistance to develop and implement all software / system. o Provide integration of Softmed system with the NMCP dictation / transcription system. o Supplied license / software subscription shall be supplied free of all additional royalties. o NMCP recognizes that the use of all supplied software is for the sole use of NMCP. NMCP agrees not to share, sell, copy, lease or assign supplied software without the expressed permission of vendor. o NMCP shall make all attempts to preserve the proprietary nature of all supplied software. Vendor shall take all reasonable measures to safeguard any and all NMCP file data. (i.e. vendor shall not share any medical records / data collected by supplied software) ?h Performance of all work shall be completed as soon as possible with a target completion date of 31 Sep 04. Note to contractor installation of NMCP dictation / transcription system is not complete at time of this award. Coordination of this requirement will be required with the installation of the dictation / transcription system. In the event that excess delays result from the installation of the dictation / transcription system, vendor must contract the NMCP contracts office with a request for a time extension. ?h The Government accepts the vendors quote as a ??Good Faith Quote?? of true cost. In the event that unexpected problems arise that reflect negatively on contract value, vendor must contact NMCP contracts office for direction, as no additional funds are available for this requirement. ?h Installed software and any supplied hardware items shall be warranted for a period of no less than one year. Vendor to provide minimum 90 day on warranty on all performed work / upgrades (software). o Warranty shall be all-inclusive against manufactures defects. o Vendor to warrant that all supplied software including downloads are free of viruses and are compatible with Naval Medical Center Portsmouth computer / hardware systems. o Vendor to provide telephone support. Support to include services for both general system operation questions and questions concerning system faults / failures.
