Description
COMBINED SYNOPSIS/SOLICITATION: AUXILIARY AIR FORCE CHAPLAIN FOR THE PETERSON AFB CHAPEL (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for proposal (RFP). Submit written proposal (to include price quote for all CLINS, base and option years) on RFP reference number FA2517-04-T-0083. (III) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. (IV) This solicitation is a small business set-aside; the associated NAICS code is 813110, and small business size standard is $6M. (V) This combined synopsis/solicitation is for the following commercial item: CLIN 0001- Weekend or Holy Day Mass: Approximately 30 (thirty) each. See SOW. Base Year Period of Performance 01 Oct 04-30 Sep 05. CLIN 0002- Weekday Mass: Approximately 58 (fifty-eight) each. See SOW. Base Year Period of Performance 01 Oct 04-30 Sep 05. CLIN 0003- Reconciliation: Approximately 26 (twenty-six) each. See SOW. Base Year Period of Performance 01 Oct 04-30 Sep 05. CLIN 0004- Penitential Service: Approximately 2 (two) each. See SOW. Base Year Period of Performance 01 Oct 04-30 Sep 05. CLIN 0005- Funeral, Wedding or Memorial Service: Approximately 6 (six) each. See SOW. Base Year Period of Performance 01 Oct 04-30 Sep 05. CLIN 0006- Cemetery Internment: Approximately 6 (six) each. See SOW. Base Year Period of Performance 01 Oct 04-30 Sep 05. CLIN 0007- Religious Instruction Class, Pastoral Counseling or Emergency Ministration: Approximately 6 (six) each. See SOW. Base Year Period of Performance 01 Oct 04-30 Sep 05. The CLIN structure (7 (seven) CLINS each year) will be the same for the 4 (four) following option years: 01 Oct 05-30 Sep 06 Option Year 1 (One) 01 Oct 06-30 Sep 07 Option Year 2 (Two) 01 Oct 07-30 Sep 08 Option Year 3 (Three) 01 Oct 08-30 Sep 09 Option Year 4 (Four) (VI) Description of requirements are as follows: (SIMPLIFIED) STATEMENT OF WORK AUXILIARY AIR FORCE CHAPLAIN RESPONSIBILITIES AND TASKS Conducts formal worship services. Administers appropriate sacramental rites and sacred sacramental ordinances. May conduct religious instruction classes. Administers emergency ministration in cases of serious illnesses, accidents, or fatalities. Provides other related duties as assigned by the Government Senior Chaplain. Is required to present valid ecclesiastical credentials (ordination certification) prior to employment. Credentials are filed with 21 SW/HC. QUALITIES: Needs to possess adaptability and flexibility. Must manifest a cooperative spirit, be able to relate to a wide spectrum of people, and be able to meet the demands of our unique parish blend of active duty and retired personnel. DUTIES AND RESPONSIBILITIES: Conducts funerals, memorial services, and internment rites. Provides pastoral counseling. Provides chaplain emergency ministration in absence of site chaplain. Administers appropriate sacramental rites and sacramental ordinances, if applicable. Provides other related duties as assigned. CONTROLS OVER WORK: Receives professional advice, guidance and direction from Government Senior Chaplain. Complies with all applicable Air Force directives such as AFPD 52-1 and AFI 52-101, which are available at http://www.e-publishing.af.mil/ and the chapel, or call (719) 556-4442 for more information. SPECIFIC TERMS OF CONTRACT It is understood that hours spent planning, preparing, consulting, rehearsing and providing will be more numerous in anticipation of and during the high Holy Seasons of Easter and Christmas. Any Auxiliary Air Force Chaplain replacement is the sole responsibility of the Auxiliary Air Force Chaplain. Auxiliary Air Force Chaplain will coordinate absences at least 30 days in advance with the Government Chaplain Advisor. Auxiliary Air Force Chaplain must keep a record of units provided and not exceed the total units authorized under the terms of this contract. Auxiliary Air Force Chaplain will provide an invoice to the Chapel Appropriated Fund Manager no later than the last day of the month. Government Chaplain Advisor will verify units provided. Statement will reflect all units completed during the month. Auxiliary Air Force Chaplain is responsible for arranging his own travel and lodging. (VII) Period of Performance is 01 Oct 2004 - 30 Sep 2009 (Basic Contract year plus Four Option Years) for service at the following address: 21 SW/HC 455 VINCENT ST, BLDG 1410 PETERSON AFB, CO 80914 (VIII) FAR 52.212-1, Instructions to Offerors ? Commercial Items (Jan 2004), applies to this acquisition. In addition to written quotes for basic and all option years, offerors are instructed to provide a capability statement describing qualifications to perform requirements listed at paragraphs V and VI. (IX) FAR 52.212-2, Evaluation -- Commercial Items (Jan 1999), applies to this acquisition. Paragraph A of this clause is completed as follows: 1) Price. 2) Capability Statement (Pass/Fail Assessment) shall be used to evaluate offers. Only those offerors receiving a ?pass? assessment will be considered for award. (X) The offeror must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (May 2004). A copy of this provision is located at http://farsite.hill.af.mil/ (XI) The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (Oct 2003), applies to this acquisition. (XII) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (Jun 2004). Additional FAR 52.212-5 clauses cited in the clause applicable to this acquisition are (b) (5)(i) (14) (16) (17) (25) and (30), (c) (1) and (2). (XIII) The following clauses are also applicable to this acquisition: 52.204-7, Central Contractor Registration; 52.252-2, Clauses Incorporated by Reference; 252.204-7004 Alt A, Required Central Contractor Registration; 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items; including 252.225-7001, Buy American Act and Balance of Payments Program, and 252.232-7003, Electronic Submission of Payment Requests. (XIV) The Government intends to award a single Firm Fixed Priced contract resulting from this solicitation.
