Description
The Wilkes Barre VAMC, Wilkes Barre PA is seeking sources to provide a comprehensive Durable Medical/?Home Medical? Equipment (DME) program servicing eligible beneficiaries residing in Tioga County, New York and 19 Pennsylvania counties: Bradford, Susquehanna, Lycoming, Sullivan, Wyoming, Lackawanna, Wayne, Pike, Union, Montour, Columbia, Luzerne, Monroe, Carbon, Northumberland, Snyder, Schuylkill, Northampton and Lehigh. <div align=center>The vendor must meet or exceed JCAHO standards.</div> The vendor will furnish all labor, materials, and equipment necessary to transport government-owned DME and accessories, including sleep apnea devices and telehealth equipment, to patients? homes, assemble DME and educate patients on proper and safe operation, and provide preventive maintenance, repairs, cleaning, and storage. The contract period is 3 January 2005 through 30 September 2005 with four one-year option periods, all subject to availability of funds. This procurement will be conducted under FAR Parts 12 and 15. <div align=center>NAICS Code is 492210; size standard is $21.5M.</div> Response to this announcement will determine if sufficient competition exists to allow a small business set-aside. Interested parties are encouraged to submit written expression of their interest in this procurement, to include factual data substantiating their business size status and relevant details concerning their ability to perform the work. Rather than using the Solicitation Mailing List option, interested parties are directed to indicate interest via email to the contracting officer listed above, using the title DME INTERESTED VENDOR, and state the company name, point of contact, and business size and status. No attachments will be accepted with this initial contact. The contracting officer will verify the authenticity of the sender and then request documentation of business size/status. The agency anticipates using the Cascade procurement strategy, as follows: any award resulting from this solicitation will be made on a competitive basis, (1) IAW FAR Subpart 19.8, to an eligible 8(a) business concern, provided that a minimum of two competitive (technical and cost) offers are received from eligible Section 8(a) concerns, offers from 8(a) concerns provide the best value IAW solicitation requirements, and offers are otherwise determined responsible and eligible to receive an award. If a minimum of two offers from eligible 8(a) concerns are not received, and/or 8(a) offers are found otherwise ineligible for award, the award for the services/supplies specified in the solicitation will be made as in (2). (2) IAW FAR Subpart 19.5, to a small business concern, provided that a minimum of two competitive (technical and cost) offers are received from qualified small business concerns, offers from small business concerns provide the best value IAW solicitation requirements, and offers are otherwise determined responsible and eligible to receive an award. If a minimum of two offers from qualified small business concerns are not received, and/or small business offers are found otherwise ineligible for award, the award will be made on the basis of full and open competition from among all responsible businesses submitting offers. <div align=center><font color=red> THIS IS NOT A REQUEST FOR PROPOSAL, AND NO SOLICITATION IS ISSUED WITH THIS ANNOUNCEMENT. </div></font>
