Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E040133, is issued as a Request for Quotes (RFQ) under test procedures of FAR 13.5. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 61171 and the business size maximum is $5.0 Million. This requirement is set aside for small businesses. This office is requesting quotes on the following items: 0001 Provide all personnel, travel, materials, and supplies required and necessary to provide a Teacher Mentor for the Northern Pueblos Agency Schools in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. Period of Performance (POP): September 15, 2004 to June 30, 2005. 0002 Option Year One Provide all personnel, travel, materials, and supplies required and necessary to provide a Teacher Mentor for the Northern Pueblos Agency Schools in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. POP: July 1, 2005 to June 30, 2006. 0003 Option Year Two. Provide all personnel, travel, materials, and supplies required and necessary to provide a Teacher Mentor for the Northern Pueblos Agency Schools in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. POP: July 1, 2006 to June 30, 2007. 0004 Option Year Three. Provide all personnel, travel, materials, and supplies required and necessary to provide a Teacher Mentor for the Northern Pueblos Agency Schools in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. POP: July 1, 2006 to June 30, 2007. 0005 Option Year Four. Provide all personnel, travel, materials, and supplies required and necessary to provide a Teacher Mentor for the Northern Pueblos Agency Schools in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. POP: July 1, 2008 to June 30, 2009. Please provide a breakdown of your costs. The Government intends to award a firm fixed price contract with a base and four option year periods. SCOPE OF WORK: The Teacher Mentor shall assist each of the following schools one day per month: (1). Te Tsu Geh Oweengeh Day School (Tesuque), Santa Fe, NM (2) San Ildefonso Day School, Santa Fe, NM (3) Santa Clara Day School, Espanola, NM (4) Ohkay Oweenge Community School, San Juan Pueblo, NM (5) Taos Day School, Taos, NM and (6) Santa Fe Indian School, Santa Fe, NM. The Teacher Mentor shall perform the following at each school: 1. Using observations, feedback and coaching work with teachers either individually or in groups to: A. Refine teachers? techniques/instructional strategies to include those that have been proven effective. B. Incorporate more hands-on learning activities. C. Refocus on instruction especially for special learners, which is meaningful conceptualization and connects new learning to their knowledge bases. D. Build a broad repertoire of approaches that have proven successful with students of varying abilities, backgrounds, and interests. E. Build teachers? knowledge base with new research findings. 2. Provide administrators (school and agency) feedback in the form of a monthly report that will assist with school improvement. Travel costs shall be reimbursed in accordance with the Federal Travel Regulations. Closing Date of Quote is September 3, 2004 9:00am local time (Albuquerque, New Mexico).
