- Offutt AFB requires the following commerical items: Survivair Quick2000 Chemical-Biological Escape Hood Respirators, Packaging: Vacuum bag plus Hard Case, Instruction Manual, bar-coding; Manufacturer: Quick Protective Systems, Inc., Part Number: 600685. 3,500 Each. Include any additional charges such as shipping on your quotations if applicable. The RFQ Number is FA4600-04-T-0167. This will be a Firm Fixed Price requirements contract. A Request for Quotes (RFQ) shall be issued on or about 02 September 2004. This acquisition is set-aside 100% for small business. Award of the contract will be made to the responsive, responsible offeror whose quotes offer the best value to the Government. Upon issue, the Request for Quotations (RFQ) will be available on the Internet at http://www.eps.gov. Contractors are required to be actively registered in the Central Contractor Registration prior to award of any Government contract. See http://www.ccr.gov.
