Description
This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Paper copies of this solicitation will not be available. This is a commercial solicitation using Simplified Acquisition Procedures found in FAR Part 13, soliciatation no. FA4690-05-Q-0004 and is issued as a request for quotes (RFQ) on 30 August 2004. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2001-24. FAR clause descriptions are available via the Internet at http://farsite.hill.af.mil/. The set-aside for this solicitation is 100% small business. NAICS code: 713990. Size Standard: $6,000,000.00. This solicitation is for sports officials. The contractor shall provide pricing for sports officiating defined in the statement of work (see attachment 1). The period of perfromance will be 1 Ocotober 2004 through 30 September 2005. FOB: Destination for delivery to Ellsworth AFB, SD 57706. The Government will award a firm-fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price factors considered. The following factors shall be used to evaluate offers: price. SUVELLANCE IS BY RANDOM SAMPLING STATEMENT OF WORK SPORTS OFFICATING INSTRUCTION C-1 GENERAL 1.1. SCOPE OF WORK. The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items or services necessary to perform sports officiating defined in the Statement of Work at Ellsworth Air Force Base South Dakota. 1.2. PERSONNEL. 1.2.1 Contract Manager. The contract shall provide a contract manager who shall be responsible for the performance of work. The name of this person and alternate(s) who shall act for the contractor when the manager is absent shall be designated in writing to the contracting office. 1.2.1.1 The contract manager?s alternate shall have full authority act for the contractor on all contract matters relating to daily operations of this contact. 1.2.1.2 The contract manager or alternate shall be available within two work days to meet on the installation with government personnel designated by the contracting officer to discuss the problem area. 1.2.2 Employees. The contractor shall ensure that employees have current and valid professional ASA certification for games they officiating and provide the proof prior to the start of the sports season. The Contractor will submit a letter to Fitness Center Director/Sport Director requesting approval of all non certificated officials. FLAG FOOTBALL (Intramural) BASKETBALL (Intramural, Varsity, Recreational) VOLLEYBALL (Intramural, Varsity, Recreational) SOFTBALL (Intramural, Varsity, Recreational) SOCCER ? OUTDOOR (Intramural, Varsity SOCCER ? INDOOR (Intramural, Varsity) 1.2.2.1 The Contracting Officer reserves the right to question any official?s certification and notify the contractor. 1.2.3 Employee Training. The contractor shall conduct a training camp prior to the start of each sporting event, updating the officials on current rules as publicized by each sporting association. The Contractor shall maintain records on each employee containing certificates of instruction and training documentation. 1.3 QUALITY ASSURANCE. The government will evaluate the contractor?s performance under this contract using the method of surveillance specified at Technical Exhibit 1. The government will record all surveillance observation. When an observation indicates defective performance, the Technical Inspector will request the contractor?s representative to initial the observation. 1.4 PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, equipment and materials shall secure. 1.5 VARSITY AND INTRAMURAL SPORTS HOURS OF OPERATION: 1.5.1 Normal hours are Monday through Thursday 0430 -2300, Friday 0430 ? 2100, Saturday 0900 ? 2100, Sunday 1100 ? 2100. 1.5.1.1 The contractor will be required to provide service based upon sport schedules provided by the fitness center. 1.5.1.2 The contractor may be required to provide service on holidays. SECTION C-2 DEFINITIONS 2.1.1 Defective Service. A service output does not meet the standard of performance. 2.1.2 Quality Assurance. Those actions taken by the government to assure services meet the requirement of the Statement of Work. 2.1.3 Technical Inspector. A government personnel responsible for surveillance of contractor performance. 2.1.4 Quality Assurance Surveillance Plan. An organized written document used for quality assurance surveillance. The document contains specific methods to perform surveillance of the contractor. 2.1.5 Quality Control. Those actions taken by the contractor to control the performance of services so that they meet the requirements of the Statement of Work. 2.1.6 Reserved 2.1.7 Exercise. The Fitness Center will participating in the Base Exercise so there will not be any games scheduled for those time. 2.1.8 Non- EAID Equipment. For the purpose of this contract this Non- Appropriated Fund Equipment that is not maintained by base supply. SECTION C-3 Government Furnished Property and Services 3.1 PROPERTY 3.1.1 Facilities. The government shall furnish and or make available facilities described in Technical Exhibit 5a. Facilities have been inspected for compliance with Base Safety. No alterations to facilities shall be made without specific written permission from the Fitness Center Director. The contractor shall return the facilities to the government in the same condition as received, fair wear and tear and approved modification accepted. The contractor shall use the facilities for the performance of this contract only 3.1.2 Equipment. The government shall provide the contractor equipment listed in Technical Exhibit 5b, 5c, and 5d. The contractor shall be responsible for maintenance and cleaning of equipment items on a daily basis. 3.1.3 Equipment Accountability. Completion or extenuation of this contract shall conduct a joint inventory of equipment conducted by the contractor and a government representative. The contractor shall be liable for loss or damage of government property beyond fair wear and tear. 3.1.3.1 Reserved 3.1.3.2 Reserved 3.2 SERVICES. The government will provide only the following services necessary. 3.2.1 Utilities. Gas, electricity, water, sewage, heating, fuels and air conditioning in support of this contract. 3.2.2 Telephone. Class A/B Telephone authorized for transaction of official government business only, which have access to on base phones and commercial toll trunks. No DSN access. 3.2.3 Custodial Service. Services shall be provided by the current custodial contract. 3.2.4 Refuse Collection. Services shall be proved by current contract. 3.2.5 Insect and Rodent Control. Services shall be provided by Civil Engineering Entomology. 3.2.6 Security Police and Fire Protection. Will be provided by the government. Phone number 911. SECTION C-4 CONTRACTOR FURNISHED ITEMS AND SERVICE 4.1 GENERAL. Except for those items and services to be furnished by the government in Section C-#, the contractor shall furnish everything required to performance this statement of work. 4.2 EQUIPMENT. Reserved. SECTION C-5 STATEMENT OF WORK OF OFFICALS 5.1 OFFICIALS REQUIREMENTS 5.1.1 Officials will be paid for games worked and forfeits announced at game time. 5.1.2 Officials will report fifteen (15) minutes prior to game time. 5.1.3 All officials will be in complete uniform for games, as specified in applicable sports rules and EAFB By-Laws and the contractor. 5.1.4 All games will be run in accordance with applicable sports rules and regulations, and the rule and regulations as modified by EAFB by-laws. 5.1.5 Games schedules and by laws for each sport will be provided to the contractor prior to the beginning of the season and updated as required by the Sport Director/Fitness Center Director. 5.1.6 The Sports Director/Fitness Center Director will furnish the contractor the names of individuals who are authorized to change the schedule or cancel scheduled games. 5.1.7 All officials will turn in a written statement for all incidents of unsportsmanlike conduct/ejection to the Sports Director or Fitness Center Director immediately following the contest. 5.1.8 Any questions or problems that may arise with in the intramural, recreational or varsity sports program will be brought to the attention of the Sports Director or Fitness Center Director. 5.1.9 All officials will attend a clinic of instruction and be administered an approved sports association test by the contractor. Test scores from other sanctioned agency will be accepted. The results of the test will be kept on file for review by the Sports Director or Fitness Center Director during the entire season. 5.1.10 Officials scorebook/score sheet will returned to the fitness center upon completion of the sporting event. Scorebooks/score sheets will have the official?s name (printed and signed) 5.1.11 For outdoor sports the officials will be responsible to ensure that all lights are off before leaving the area. 5.1.12 Make up games will be rescheduled at the discretion of the Sports Director. If the games are to be played the date and time will be coordinated with the contractor 5.1.13 Personnel designated as umpire/officials will be authorized access to EAFB for the sport. 5.1.14 The Bellamy Fitness Center Sports Director has the authority to report all conduct by an official which he/her considers detrimental to the program to the contractor for corrective action. 5.1.15 Officials will pick up the necessary equipment required to the sport and insure that all equipment is returned to the Bellamy Fitness Center. 5.1.16 Contractor will provide the Sport Director a list of officials that are scheduled to work during a p particular week by noon on Monday of that work week. 5.1.17 Officials will sign the Officials Pay Sheet upon completions of the sporting event. 5.1.18 Sport Director will approve the officials/umpires for all tournaments 5.1.19 If games are canceled due to officials not showing up or being more than 15 minutes late the normal rate for on games will deduct from the payment. 5.1.20 Contractor will evaluate all officials with the first two weeks and mid point of the season. Officials hired after the season has started will be evaluated with the first week and at the mid point. Contractor will conduct evaluation of officials when requested by the Sports Director. Contractor will furnish a copy of all evaluation to the Sports Director. 5.1.21 Contractor will furnish the name, duty phone and home phone of all officials working that sport. 5.2 SAFETY. Officials shall ensure that safety procedures are followed in accordance with the governing rule book and local by laws. SECTION C-6 APPLICABLE PUBLICATION AND FORMS RESERVED TECHNICAL EXHIBIT 1 CONTRACT STANDARDS The contractor shall provide all personnel, and supervision necessary for officiating at Ellsworth AFB SD. Any part of the service that is not in agreement with the performance requirements shall be declared defective in quality. The contractor without additional cost to the government shall correct unsatisfactory work. Payment will not be made for services not received or for unsatisfactory service. The contractor will be required to provide a price list for individual at the start of the contract.
