Description

This is a solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.. A written solicitation will be issued. Solicitation number 673-02-05 applies and is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. The North American Industrial Classification Code is 492210 and the business size standard is 1,500 employees.. Provide all supervision, labor, and material to provide delivery of lab specimens, medical records, and x-ray films from Orlando VA Healthcare Center, Orlando, FL to Sanford, FL in accordance with the terms and provisions of the solicitation for the period October 1, 2004 thru August 21, 2005 with option to extend four additional years. Service will originate and terminate at the Orlando VA Healthcare Center, 5201 Raymond Street, Orlando, FL. Note # 1 applies. Subject to availability of funds. All quotations are to be submitted by hard copy to the designated office by COB September 15, 2004. Faxed versions are acceptable. All other electronic versions are not acceptable. Electronic version and hard copy of solicitation will be available August 30, 2004. Electronic request for quotation may be obtained free at the following URL:http://www.fedbizopps.gov . Any amendments or additional information issued to this request for quotation will also be available electronically at the URL address. Request for hard copy solicitation must be in writing. No telephone request will be accepted. Only written or faxed request received directly from the requestor no later than September 9, 2004 are acceptable.
