- NAICS Code: 213112 Plug abandoned wells on Osage Land, in accordance with the Statement of Work, Region 6 EPA Staff will conduct a site visit at the Osage Tribal Complex in Pawhuska, OK, on September 02, 2004, at 10:00 AM., all questions or concerns about the work to be done will be addressed at the site vist only. any other questions or concerns prior to or after the site wist will be addressed to Diane Terry via Email at terry.diane@epa.gov. Statement of Work available upon request. Contractor shall sumbit certification that they will perform all requirements as set forth in the Statement of Work or provide any exceptions in writing with specific detail to exceptions of the Statement of Work. Award will be made to the technically acceptable offeror providing lowest price. After the site vist please prepare a price quote and forward to Diane Terry at EPA, 1445 Ross Ave., Suite 1200, Dallas, Texas 75202 or Fax to (214) 665-8505, Attn: Diane Terry. on or before September 09, 2004.
