- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number M68909-04-Q-1097 is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. This is a small business set-aside. SIC 1771, Size 500 Employees. The contractor shall provide all labor and equipment for CLIN 0001: Concrete, 2415 sq ft, minimum 6? thick, and maximum 13? thick, 3000 PSI with # 5 grade 60 rebar. The concrete shall be poured around, and matched evenly with an existing 1,716 sq ft slab of concrete. CLIN 0002: Fill in gap (1? or less) from concrete to asphalt with asphalt. CL! IN 0003 Sealant for the entire (4131 sq ft) concrete area. Anchor bolts, provided by the Government, shall be placed as shown on blueprints (provided upon request) at a depth of at least 13?. CLIN 0004 Repair 60ft long edge of existing concrete slab, which is pitted and chipped. Concrete work shall take place at the Marine Corps Tactical Systems Support Activity (MCTSSA), Camp Pendleton, California. The contractor shall dispose of all rubbish and debris from Government property as required in accordance with Marine Corps policies. The concrete shall be completely poured, finished, and ready for use no later fourteen (14) calendar days after contract award. Standard commercial warranty shall apply. The provisions at FAR 52.212-1, Instructions to Offerors?Commercial Items and FAR 52.212-2, Evaluation?Commercial Items shall apply. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation! will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: Price, delivery, and past performance. The offerors shall provide names, contact points, and telephone numbers of three (3) commercial or government customers that are currently using the items being offered for purposes of past performance evaluation. Offerors shall include a completed copy of the provision at FAR 52-212-3, Offeror Representations and Certifications?Commercial Items, with its offer. The provision at FAR 52.212-4, Contract Terms and Conditions?Commercial Items, apply to this acquisition. The provision at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items, Subparagraphs (a), (b) (5), (6), (7), (11), (12), (14), (15), (d) and (e) apply to this acquisition and are hereby incorporated by reference. The provision at DFAR 212-301 Solicitation Provisions and Contr! act Clauses for the Acquisition of Commercial Items, subparagraphs (f)(ii) and (f)(iii) apply to this acquisition and are hereby incorporated by reference.
