- Under the authority of FAR Part 13.5,Test Program for Certain Commercial Items, VA Medical Center located at 7305 N. Military Trail West Palm Beach, Florida 33410-6400 has a requirement to establish a firm fixed price, indefinite delivery, indefinite quantity contract for dental laboratory services to include, but not be limited to, crowns, retainers and semi and fully adjustable articulators. Contractor shall furnish all personnel, equipment, materials, supplies, transportation and supervision necessary to provide services, as detailed in the Schedule of Supplies/Services, and in accordance with the terms and conditions of this solicitation and the Statement of Work. Contractor shall provide Certified Dental Laboratory Technician (CDT) with minimum of five (5) years experience. Dental Laboratory must be actively registered with the Florida Department of Health. Contractor shall be located within twenty (20) mile radius of VA Medical Center or thirty (30) minute driving distance, to allow for direct supervision of Contractor by one of VA Prosthodontists. Direct supervision is necessary for some complex cases which involve hands interaction of Prosthodontist and Dental Lab. Technician when designing, waxing, casting or finishing implant superstructures and related metal frameworks. Laboratory cases shall be picked up from and delivered to Dental Service at VAMC, West Palm Beach, Florida. Period of performance shall be 10/1/04 ? 9/30/05 with two option years for renewal. Contractor shall furnish services Monday ? Friday, 8:00 am ? 4:00pm, excluding Federal holidays. Contractor shall respond to VA call for pick up within twenty-four (24) hours of verbal request and deliver back to VAMC, WPB within fifteen (15) business days. For cases determined by VA Medical Center to be emergent, Contractor shall provide a four (4) hour turn around time from pick up to delivery. No telephone or fax requests for solicitation will be accepted. Any and all questions regarding the solicitation shall be faxed to 561-422-6507. Solicitation will issue on or about 9/1/04. Response date will be on or about 9/15/04.
