- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Proposals must be received no later than the bid close date. See attached Statement of Work, “SOW For Replacing Deionizer Water System in Building 238” for detailed description of services required. APPROXIMATE SOLICITATION ISSUE DATE: None – see paragraph above. Contract Line Item Structure (CLIN), (Please propose according to the following structure): CLIN 0001: Removal and Installation of a DI Water System in Bldg. 238. To include all labor, materials, personnel, and be done in accordance with statement of work as incorporated. This system must be “turn-key” ready to go with all salt, DI bottles, membranes, and operating supplies fully installed, and ready for use. Please provide a total price, to be paid upon acceptance of system. Period of Performance: Not to exceed three months from date of contract award. Unit of Measure: Lot, Qty: 1. CLIN 0002: Monthly/emergency maintenance of the above system. The contractor will be responsible for all maintenance – required, regularly scheduled, and emergency servicing for a period of 12 months after the acceptance of the system (CLIN 0001). This will include everything except the cost of DI bottles and salt. Please provide a set monthly price. Period of Performance: Twelve months from government acceptance of CLIN 0001. Unit of measure: Month, Qty: 12. CLIN 0003: Materials - Salt and DI Bottles needed after initial acceptance of the system, not related to defects or failure to comply with CLIN 0001. This line item will be cost-reimburseable only. Please provide an estimate-only. Period of Performance: Twelve months from government acceptance of CLIN 0001. Unit of Measure: Lot, Qty. 1. APPROXIMATE AWARD DATE: 27 September 2004 Clauses/Provision: The solicitation document and incorporated provisions and clauses are those in effect through the Standard Form 1449 Commercial Contract. This includes, but is not limited to the following provisions: 52.212-1, Instructions to Offerors – Commercial; 52.212-4, Contract Terms and Conditions – Commercial Items; 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders – Commercial Items; and 52.212-3 Offeror Representations and Certifications-Commercial Items. The offeror must include a completed copy of the provision at 52.212-3 (attached), Offeror Representations and Certifications – Commercial Items, with its offer. Place of Contract Performance/Delivery: Hill AFB, UT, period of performance is to be for one-year. Acceptance will be by government personnel. This acquisition is full and open competition.
