- FA8601-04-T-0156, SYNOPSITATION FOR CCTV SYSTEM COMPONENTS This is a combined synopsis/solicitation for a commercial item to be procured for the New Horizons and Wright Care Child Care Centers at Wright Patterson Air Force Base, Ohio, prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA8601-04-T-0156 is issued as a Request for Quotation (RFQ), CLOSING DATE AND TIME ARE 8 SEP 04 AT 4:00 PM EST. No hard copies will be sent. THIS PROPOSED ACQUISITION IS A 100% SMALL BUSINESS SET-ASIDE. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. The associated Standard Industrial Classification (SIC) code is 7382 and the North American Industry Classification System (NAICS) code is 561621. The small business size standard is $10.5 Million. Resultant contract will have the following structure. All quotes must be broken down and prices shown for each Contract Line Item Number (CLIN) below. A complete description of services/materials to be provided is contained in the attached Statement of Work. CLIN 0001: 4 each - 40? LCD Flat Screen Monitors CLIN 0002: 4 each - Wall mounts for 40? LCD Flat Screen Monitors CLIN 0003: 3 each - 30? LCD Flat Screen Monitors CLIN 0004: 3 each - Wall mounts for 30? LCD Flat Screen Monitors CLIN 0005: 1 each - Digital Video Recorder CLIN 0006: 1 each - Distribution Amplifier (16 x 4) CLIN 0007: 1 each ? Delivery/Installation and Training (includes all miscellaneous hardware and accessories necessary for proper installation including power strips, mini patch BNC-BNC connectors, etc.) Solicitation provision at FAR 52.212-1, Instructions to Offerors ? Commercial (Jan 2004) is hereby incorporated by reference. Award will be based upon best value to the government considering cost and other factors, see FAR 52.212-2 ? Evaluation ? Commercial Items (Jan 1999). The following factors shall be used to evaluate offers: (1)Price, (2) past performance. ALL OFFERS SHALL INCLUDE A COMPLETED COPY OF PROVISION 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATION COMMERCIAL ITEMS (MAY 2004) (ATTACHED). OFFERS MUST ALSO INCLUDE THREE REFERENCES (INCLUDE NAMES AND PHONE NUMBERS). The Government may contact any or all of the references for past performance information. This information may then be used in determining who will be awarded any resultant contract. Solicitation provision at FAR 52.237-1, Site Visit, is hereby incorporated by reference. A site visit is scheduled for 1 Sep 04 at 0900 hours. All interested parties must notify Helen Williams by 30 Aug 04 that they will be attending the site visit. The names and Social Security Numbers of persons attending must be provided (limit 2 persons per company). Attendees will meet in the Hope Hotel and Conference Center Parking Lot inside Gate 12A, Wright- Patterson AFB, Ohio, at 0845 hours. From there transportation will be provided to locations where work will be performed. Section 1008 of the National Defense Authorization Act of Fiscal Year 2001 requires any claims for payments (invoices) under DoD contract to be submitted in electronic form. Wide Area Workflow-Receipt and Acceptance (WAWF- RA) is the DoD system of choice for implementing this statutory requirement. Use of the basic system is at no cost to the contractor.
