Description
The Department of Veterans Affairs (VA) Medical Center, San Juan, Puerto Rico intends to award a commercial item (FAR Part 12) Brand Name orEqual (Far 52.211-6)contract for the Vectorvision Platform that is an ear, nose and throat (ENT) Image Platform for cranial and CT-fluoro Spine procedures to support the neurosurgery services. This is a combined synopsis/solicitation for Commercial Items prepared in accordance with the format in the Federal Acquisition Regulation (FAR), Subpart 12.6 as supplemented with additional information included in this notice. This platform must: be capable to provide guidance and transfer data in/out with a scanner and easily network accesss with a fast visualization images of the surgery procedures; have more than one function including a cranial software and a spine software with all components for the surgery procedures; be able to transfer and copy patient data in/out; track all data and 2D/3D visualization with multiple displays of the instruments procedures; include all necessary registrations and be easily used for training and clinical purposes; be able to be used for more than one medical department and medicalpersonnel; provide accurate localization of the surgical site that avoids brain and orbital complications and the system must be similar to Global Positional System (GPS).The Company must be specific on warranties,installation,training services and any other requirements for this equipment. This announcement constitutes the only solicitation.Quotations are being requested and a written solicitation will not be issued. Solicitation Number is RFQ 672-377-04. FAR 52.212-1 Instructions to Offerors-Commercial Items (Jan 2004),52.212-3 Offeror Representations and Certifications-Commercial Items(May 2004), 52.212-4, Contract Terms and Conditions- CommercialItems (Oct 2004), FAR 52.2 1 2-5 Contract Terms and Conditions- Commercial Items (June 2004),Contract Terms and Conditions required to Implement Status or Executive Orders-Commercial Items, Far 52.247-64 Preference for privately Owned U.S.-Flag Commercial Vessels are applicable clauses. The Government will award to the lowest technically acceptable proposal. The proposal must be evaluated as technically acceptable for each of the requirement listed above. Technical acceptability will be determined solely on the content and merit of the information submitted in response to this solicitation the "Equal" for the Service evaluation and decision. The applicable NAICS is 339111. Total Price$__________. Delivery Terms: FOB Destination (Door to Door) ?52.232-33 Payment by Electroninc Funds Transfer. The Contractor shall deliver the platform within 30 days or less from the date of contract award to Department of Veterans Affairs V A Medical Center, Warehouse loading Dock Area; 10 Calle Casia, San Juan, Puerto Rico 009213201.Inspection and acceptance shall be made at destination by the Goverment.46.709 Warranty of Commecial items -The Contractor shall extend to the government full coverage of any standard commercial warranty normally offered in a similar commercial sale. Acceptance of the warranty does not waive the Government's rights under the Inspection clause, nor does it limit the Government's rights with regard to the other terms and conditions of this contract. In the event of a conflict the terms and conditions of the contract shall take precedence over the warranty. The Government contemplates award of a Firm Fixed-Price contract resulting from this solicitation.
