Description

This is a combined synopsis/solicitation for commercial items prepared in accordance witht he format in subpart 12.6, as supplemented with addtional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicatiation will not be issued. Medical Courier Services: Contractor shall provide regularly scheduled pick-ups of test specimens and prescription forms from two VA Primary Care Centers and one Outpatient Clinic listed in the SOW and deliverd to VA Medical Center, Providence, RI, all in accordance with the requirements of the sow. Contractor personnel must be trained in "Universal Precautions" in handling of biohazard materials. Contractor must be a courier company with state(s) license(s)of MA & RI, carry a minimum of $1,000,000 liability insurance, and must be bonded. All drivers must carry cell phones or pagers while deliveries are in progress. Any delays in specimen deliveries must be reported to the Main Laboratory in Providence, RI.
