- Description
- VA Medical Center, Minneapolis, MN is seeking offerors to provide Community Hospice Care for eligible veterans located in thirty-seven counties of Minnesota; fifteen (15) counties located in Wisconsin and two counties located in Iowa. Offeror must be according to Hospice Standards Medicare certified and licensed by the state/states in which service is provided and remain in compliance throughout the duration of the contract. This procurement is being conducted under a combination of Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items and Part 13 Simplified Acquisition Procedures and the procurement of Hospice Resources are issued under Title 38 United States Code Section 1717 and 1720C, with a Commercial Items format of terms, conditions, and provisions/clauses. NAICS Code 621999 applies for this procurement and the small business size standard is $8.5 million. Multiple contracts will be awarded for the period of October 1, 2004 through September 30, 2005 with four one-year options to renew, subject to the availability of funds. Proposal documents are available for download from the BOS web site. Electronic offers will not be accepted.
