Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. STATEMENT OF WORK FOR THE FORT MONMOUTH PHYSICAL FITNESS CENTER AEROBIC PROGRAM 1. The contractor shall provide instruction to patrons to include (but not limited to) movements and techniques normally acceptable in an Aerobic Exercise Class. 2. The contractor will provide her/his own music for the course. The equipment on which to play the music will be provided by the Fort Monmouth Physical Fitness Center. 3. The instructor is responsible for replacing and securing any Fort Monmouth Morale Welfare& Recreation Facility (FMMWRF) supplies and equipment used for instructional purposes. 4. The instructor will ensure all equipment used during the lessons is stored properly and accounted for. 5. The Contracting Officer Representative (COR) has the authority to change dates, times per week, and location of the classes to accommodate changes in facility and/or user needs. This includes (but is not limited to) make-up classes. 6. Participants will not receive any completion of certification or class completion certificates. 7. Classes will be cancelled and rescheduled if at all possible if Fort Monmouth is closed due to circumstances beyond their control. In the event no class can be held because no students show up, the contractor shall receive full pay provided she/he was present and ready to give the class. 8. The contractor will operate at her/his own risk and must have personal insurance coverage and hold Fort Monmouth harmless from ALL liability. 9. FMMWRF will provide adequate space and equipment for the operation of the class. 10. Any act of vandalism to occupied area discovered by the contractor shall be reported immediately to the COR and to the Contracting Officer. 11. The contractor shall deliver to COR, evidence of certification, license, and/or franchise for any instructor performing under this contract. 12. If the instructor is unable to perform for any reason (illness, vacation, etc.), the contractor will subcontract a qualified instructor to perform. Contractor will be responsible for payment to subcontractor. FORT MONMOUTH PHYSICAL FITNESS CENTER AEROBIC PROGRAM 13. Classes will not be conducted on the following National Holidays: President's Day, Memorial Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, Christmas Day, New Year's Day, Martin Luther King, Jr.'s Birthday, July Fourth, and any other ho liday which falls Monday through Friday. 14. Contractor will send an invoice once a month and SELHI form 1471, Acceptance Service Statement, will be used for payment. 15. Classes will be scheduled Monday through Friday at times most convenient to the patron at the Fort Monmouth Physical Fitness Center and may be held on weekends if the demand arises. 16. Any additions or amendments to the class frequency or schedule will be determined by the COR. 17. The contractor shall be responsible for all promotional, advertisements, distribution of class schedules, and any other correspondence needed that pertain to the classes being paid-by under this contract. 18. The contractor shall give Fitness Expertise as needed to the Fort Monmouth Physical Fitness Center staff and/or patrons. If any expertise is given in a class-like or group-like atmosphere with an anticipated date and time, the contractor's monthly inv oice will consider the expertise given as part of the normal Aerobics classes given. 19. The contractor will be responsible to punch holes in every patron card. If the patron fails to present their aerobic card, they are ineligible to participate in that class. 20. The contractor will also be responsible to ch eck the expiration date, and if the card is expired, the patron must purchase a new aerobic card before they can participate in the class. (3-month time limit from the day of purchase) 21. Any patrons that lose their card, the contractor will let them know that they will have to purchase a new aerobic card at the front desk before participating in class. 22. At the end of each class, the contractor will report to the front desk to let the staff know the number of patrons that attended the class. Aerobic Instructor Criteria: 1. Contractor shall have certifications recognized by a national organization. For example: American College of Sports Medicine (ACSM); American Council on Exercise (ACE); International Dance Exercise Association (IDEA); Aerobics and Fitness Association of America (AFAA); Cooper Clinic/Aerobic Institute; National Dance-Exercise Instructor's Training Association (NCEITA); Exercise and Safety Association (ESA); American Aerobic Association International/International Sports Medicine Association (AAAI/ISMA); Aerobic Pipeline International (API). 2. Instructor must maintain current CPR and First Aid Certificates. 3. Contractor shall be able to teach a variety of classes to include: Pilates, Strength, Cardio-kickboxing, Stretching, Resist-a-Ball, Water Fitness, etc. 4. Contractor shall be able to communicate clearly with class participants. The instructor shall be able to interact with class participants. 5. Contractor will be able to perform exercise routines and also instruct participants. This means that the instructor will have an excellent level of fitness. 6. Contractor will be able to answer fitness/ wellness question should they arise. 7. Contractor will have a professional approach and appearance. Instructor will wear appropriate athletic attire for mixed-gender classes. No see-through clothing, tongs without leggings or shorts underneath, street clothes, jeans or offensive logos allow ed.
