Description

This solicitation is for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. The intent of this solicitation is to furnish Dental Laboratory Services for Crown & Bridge procedures to the Department of Veterans Affairs Medical Center, Fayetteville, North Carolina. The solicitation is being issued as a (RFP). The solicitation number is RFP #246-05-01612. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14. This is a 100% Total Small Business Set-Aside. The NAICS code is 339116 and the small business size standard is 500 employees. The VA Medical Center, Fayetteville, N.C. intends to solicit quotes and award a firm fixed price contract on a competitive basis to the offeror(s) whose quotation meets the requirements of this solicitation. Period of contract will be from date of award through September 30, 2005 with four one-year options to extend.
