- Brooke Army Medical Center, Fort Sam Houston, TX has a requirement for the purchase of two types of labels. Samples will be provided upon request. Period of performance is from 01 Oct 04 through 30 Sep 04 with four option years. This requirement w ill be on an as needed basis. The suggested source of supply is Tri-State Hospital Supply Brand name or equal. This procurement is being conducted as a commercial item acquisition in accordance with FAR Part 12. This requirement is 100% Set Aside for Sm all Business. All responsible sources may submit an offer, which will be considered. Solicitation Number W81K00-04-T-0400 will be issued on or about 24 Aug 04 and will be available at https://acquisition.army.mil/asfi/gov_personnel_main.cfm.
