This is a combined synopsis and solicitation for Missile Alert Facility Refuse Collection: ///Item 0001 ? Non-personal Services: Refuse Collection for 15 Missile Alert Facilities. Period of service from 1 Oct 2004 through 30 Sep 2005 in accordance with attached Statement of Work ///Item 0002 ? Unscheduled movement of refuse containers, 1 Oct 2004 through 30 Sep 2005, estimated 30 each ///Item 0003 ? Option Year 1 - Non-personal Services: Refuse Collection for 15 Missile Alert Facilities. Period of service from 1 Oct 2005 through 30 Sep 2006 in accordance with attached Statement of Work ///Item 0004 ? Option Year 1 - Unscheduled movement of refuse containers, 1 Oct 2005 through 30 Sep 2006, estimated 30 each ///Item 0005 ? Option Year 2 - Non-personal Services: Refuse Collection for 15 Missile Alert Facilities. Option Year 4 - Unscheduled movement of refuse containers, 1 Oct 2008 through 30 Sep 2009, estimated 30 each This combined synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice and is current to include FAC 2001-24. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). Submit written offers (oral offers will not be accepted), on RFQ F5OGFM42030200. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). Quotes will be evaluated on price related factors only. The North American Industrial Classification Standard is 562111; the small business size standard is $10.5M. This acquisition is 100% set aside for small business concerns. The following provisions and clause apply to this acquisition: FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this solicitation. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this solicitation and the offeror must include a completed copy of this provision with their proposal.
