Synopsis Classification Code-S?XUtilities and Housekeeping Services Description This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a separate written solicitation will not be issued. Solicitation Number RFQ CC-04-15 applies and is issued as a request for Quotation. This procurement is a 100% Small Business Set-aside. Attachments shall be forwarded to offeror by request. This solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-24 dated June 18, 2004 and NIH Policy Manual Issuance 6012-26012-1 Offerors are required to request attachments from the Contracting Officer, jfriedhoff @cc.nih.gov. The North American Industrial Classification System (NAICS) code is 812332 and the business size standard is 12 million dollars The Warren Grant Magnuson Clinical Center at the National Institutes of Health intends to purchase Laundry Services for Operating Room (OR) Linen and Scrubs for the Department of Anesthesia and Surgical Services for a period of twelve months-from September 15, 2004 through September 14, 2005 with one twelve month option available for unilateral exercise by the Government. This acquisition is for a fixed price requirements purchase order for laundry services in accordance with the following attachments: RFQ CC-04-15-Statement of Work and Provisions; Attach 1 is the Statement of Work; Attach 2 is the Clinical Center Housekeeping and Fabric Care Department Universal Precaution and Other Isolation Precautions; Attach 3 is Wage Determination No. 1994-2103 Rev. No. 32 dated 05/27/2004; Attach 4 is Infectious Disease Compliance; Attach 5 is Invoice Instructions NIH RC-2 Attachment 6 is Security Information (located at http://security.nih.gov/); http://forms.cit.nih.gov-Click on PDF Forms. Attach 7 ACH Form; Estimated quantities for a one year period will include OR Scrub Shirts, ea 12,500; OR Scrub Pants ea 12,500; OR Linen 18,000 lbs; The estimated quantities for the line items are exactly the same in the base year, and the option year. Parties responding to this solicitation may submit their offer in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc) but must include the following information: 1) company??s complete mailing and remittance addresses 2) discounts for prompt payment if applicable; 3) Dun & Bradstreet number; 4) Taxpayer ID number; 5)) Response to the Technical Evaluation in Section E.2 of Attachment 1. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications/Commercial Items with their offer. The Government intends to make a single award to the eligible, responsible offeror whose offer conforms to the solicitation and is the most advantageous to the Government considering Technical, Price and Past Performance. The major evaluation factors for this solicitation include technical (which encompasses experience and past performance factors) and cost/price factors. Although technical factors are of paramount consideration in the award of the contract, cost/price is also important to the overall contract award decision. All evaluation factors other than cost or price when combined are significantly more important than cost or price. In any case, the Government reserves the right to make an award to that offeror whose quotation provides the best overall value to the Government. Section B.1-Supplies or Services and Price/Costs Section B.1-Supplies/Services Article B.1 Brief Description of Supplies or Services Provide surgical scrubs and linen to the Department of Anesthesia and Surgical Services at the National Institutes of Health in accordance with the specifications herein. The contractor shall furnish all labor services, qualified and technical personnel, equipment, and facilities not otherwise provided by the Government under the terms of this order as necessary to perform the work set forth below. Article B.2 Prices/Costs The pricing/costs schedule shall be for an interim period of one year with a one year option. The offeror shall propose to the estimated quantities based on the Statement of Work. The offeror shall propose fixed unit prices for all items listed in Section B for the Performance Period. The final order will contain price/cost provisions agreed upon by the Government and the Offeror based on the estimated quantities shown.
