Description
The U S Department of Agriculture (USDA), Animal and Plant Health Inspection (APHIS), Veterinary Services (VS) is soliciting submittals from qualified health care providers to administer rabies vaccinations on an as needed basis to non-federal employees participating in the USDA National BSE Surveillance Program at risk for contracting rabies. A blanket purchase order (BPA)is anticipated with a national health care provider, however, USDA reserves the right to award multiple BPA?s if determined to be in the best interest of the USDA based on availability of rabies vaccine, boosters and titers. The effective period of the BPA(s) shall be from date of award through 12 months. Health care providers shall comply with applicable OSHA regulations for administering rabies vaccinations and boosters. The prospective provider will be required to (1) Establish local health care providers nationwide at locations designated by the USDA Contracting Officer. (2) Manage and provide oversight of local health care providers to ensure availability of specified rabies vaccine and compliance with the terms and conditions specified herein. (3) Schedule appointment to begin administering a series of pre-exposure rabies vaccinations, titer checks, post exposure immunizations if necessary, and/or boosters for authorized non-federal employees within 3 business days of receiving telephone request for appointment; (4) Advise patients on personal protective measures available to protect individuals against rabies as well as the potential risks and side effects of rabies vaccination. Provide each patient with an information pamphlet or brochure informing them about rabies symptoms, how it is transmitted, prevention, pre-exposure prophylaxis, purpose, options and possible side effects, boosters, post-exposure prophylaxis, etc. (5) Determine the status of rabies antibody titers for individuals who have previously been vaccinated for rabies and provide boosters when necessary. (6) Provide monthly report to the USDA, APHIS Contracting Officers Representative at each location of the number of primary vaccinations series, boosters, and their titer checks administered during the month. Include the name of the non-federal employee, the name of their employer, city and state of health care provider and the date of vaccination, booster or titer check was administered. (7) Provide itemized monthly invoice in arrears for vaccinations/boosters administered during the month. It is estimated that rabies vaccinations and/or boosters may be required nationwide for at least 600 non-federal employees. Submittals should include all cost associated with providing this service. Prospective providers must be registered with the Central Contractors Registration System (CCR) prior to award (www.ccr.gov). Submittals should include provider?s Taxpayer Indentification Number (TIN) and Dun & Bradstreet number (DUNS), failure to provide this information or register with the CCR will render a submittal non-responsive. SUBMITTALS ARE DUE AUGUST 19, 2004 BY 12:00 P. M. CENTRAL TIME. Submit written or faxed submittals to Purchasing Section Attn: Gloria Benson at (612)370-2136. Submittals from potential providers should address the nature of the organizations facilities, where the organization is able to provide services i.e. city, state, and region. Submittals should also provide estimates for rabies vaccinations, boosters and titers as well as all fees and taxes for individual locations.
