Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The purchase requirement number is F73SGL416103 and the solicitation is issued as a request for quotation (RFQ). This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular 01-13. This solicitation is 100% set aside for small business concerns. The contract type will be Firm Fixed Price (FFP). The associated NAICS code is 561310, and the small business size standard is $6,000,000. This requirement consists of one line item to be priced for a twelve month period: Line Item 0100: Certified Coder to perform ambulatory coding asssessments, implement data collection tools, etc. at the 43D Medical Group, Pope AFB NC as described in the Statement of Work (SOW). The period of performance is 01 Oct 04 through 30 Sep 05. Please note, funds are not currently available, as this is a FY05 requirement. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://farsite.hill.af.mil. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. The following FAR clauses identified at paragraph b of FAR 52.212-5 are considered checked and are applicable to this acquisition: (13-19), (24), (29), and C(1) and C(2). The provision at 252.232-7003, Electronic Submission of Payment Requests, applies to this acquisition. Award can only be made to contractors registered in the Central Contractor Registration web page, http://www.ccr.gov. Commerce Business Daily numbered note 1 applies to this acquisition. The solicitation due date is 14 May 04, 4:00 PM EST. The Government will award a contract resulting from this combined synopsis/solicitation to the responsible contractor whose offer conforms to this combined synopsis/soliciation, who has the technical ability to meet all requirements outlined in the SOW, and will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Price (2) Past performance. Past performance is significantly more important than cost or price. The Government will evaluate the quality and extent of offeror?s experience deemed relevant to the requirements of this RFQ. The Government will use information submitted by the offeror and other sources such as other Federal Government offices and commercial sources, to assess experience. Provide a list of no more than three (3) of the most relevant contracts performed for Federal agencies and commercial customers within the last 3 years. Relevant contracts are defined as those that are similar in nature to the proposed effort, Medical Administrative Support Services. Furnish the following information for each contract referenced: (1) Company/Division name (2) Product/Service (3) Contracting Agency (4) Contract Number (5) Contract Dollar Value (6) Period of Performance (7) Name, Address, FAX & phone number of the Contracting Officer. AFFARS 5352.223-9002, Requirements Affecting Contractor Personnel Performing Mission Essential Services also applies to this requirement In addition, Noncompete Provision In Contractor?s Employment Agreements applies to this requirement. This provision reads as follows: A non compete condition in the contractor?s employment agreement with its employees that prevents employees of the incumbent from being employed, or accepting offers of employment, by the new contractor on the follow-on task order hinders the government?s ability to accomplish the mission of providing continuity of quality medical care to beneficiaries.
