- This is a combined synopsis/solicitation, Request for Quotation 0401-4-0031, for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented without additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 19. This is 100 percent small business set-aside. The NAIC code is 212321 the size standard for this code is 500 employees. South Side Road Aggregate. The project is located on the Ferron Ranger District of the Manti La-Sal National Forest and consists of delivery and tailgate/windrow spread of 4650 tons of aggregate surfacing that meets Forest Service specification grading F, (Forest Service Specifications for Construction for Roads and Bridges Section 304-Aggregate Base or Surface Course) to the South Side Road, #50043. Spread lengths will be designated on the ground by the Forest Service. The aggregate will be shaped by Forest Service Crews. The segment receiving aggregate will be from Wrigley Reservoir westerly to the intersection with the Sage Flat Road a distance of approximately 3 miles. Special project specifications are as follows: DESCRIPTION 304.01 Work. Delete and add the following: Deliver and windrow spread aggregate surface course on the prepared subgrade.Certified weight tickets are to be provided to Forest Service personnel on site. MATERIALS. 304.02 Source. Delete.304.03 Gradation. Delete and add the following: Ensure that grading requirements for crushing or screening operations meet the requirements specified in Subsection 703.05.304.05 through 304.11 delete 304.12 Delete and add the following: Delivery. Deliver aggregate Monday through Thursday, 7 AM to 5 PM, 665 tons per day. Place aggregate in windrows based on spread lengths designated on the ground. Operate the hauling equipment over the previously placed aggregate in a dispersed manner to minimize rutting or uneven compaction. PAYMENT. 304.13 Basis Pay Item Pay Unit 304(11) Crushed aggregate, surfacing, grading F, English weight units Tons. DELIVERY: The project can be reached by heading west out of Ferron, Utah on County Road 701/Forest Road 50022 towards Millsite Reservoir. Follow this road approximately 14 miles to the intersection with the South Side Road 50043. Take the South Side Road approximately 2.3 miles to Wrigley Reservoir and the start of the project. The provisions of 52.212-1, Instructions to Offerors-Commercial; 52.212-2 Evaluation-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items apply to the RFQ. Evaluation Criteria to be used in FAR 52.212-2 Evaluation-Commercial items are (1) past performance specifications (2) price. Vendors must register in the Central Contractors Registration (CCR) in order to be awarded a contract. See http://www.ccr.gov/handbook.cfm for a template of information necessary for registration.
