Description

The Contractor shall provide non-emergent ground transportation twenty-four hours per day, six days a week, Monday through Saturday to I.H.S. patients who are authorized by the NNMC CHS staff. Patients shall be transported from and to home of residence to dialysis center, other IHS facilities and non-I.H.S. facilities. The Contractor shall provide transportation with the states of New Mexico, Colorado, Utah, and Arizona.Anticipated award will be from date of award thru 30 September 2005 with two one year option periods. Interested offeror(s) should submit their request for RFPM04-0019PD, closing date on September 08, 2004 at 4:00 p.m. local time.
