- The 20th Contracting Squadron, Shaw AFB, S.C. is requesting a quote on the following: 1ea-Everest VIT VideoProbe XL Pro part number PXLA726ASYS; 1 ea-Tip Optic part number PXT7100FG; 1 ea-Tip Optic part number PXT7100SG; 1 ea-Retrieval Tool Kit part number WT-726KIT; 1 ea-Battery and Charger set part number XA207S-XL; 1 ea-VideoProbe XL Pro part number PXLA415A UNT; 1 ea-Tip Optic part number PXT480SG; 1 ea- Magic Arm-Mini part number XA201. Contractor shall perform all equipment set-up, maintenance, and proper-handling training upon receipt of items solicited. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This requirement is set aside 100% for small businesses. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotation (RFQ) # FA4803-04-QS296. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-22. The North American Industry Classification System (NAICS) code is 334519, and the size standard is 500 employees. Only faxed or e-mailed quotations will be accepted. Submissions must include a completed copy of the provision at *FAR 52.212-3, Offeror Representations and Certification ? Commercial Items, name, address, cage code, DUNS number, and telephone number with its offer. In addition, the following provisions and clauses apply to this acquisition: *FAR 52.212-1, Instructions to Offerors Commercial; *FAR 52.212-2, Evaluation ? Commercial Items (Price and Delivery); *FAR 52.212-4, Contract Terms and Conditions-Commercial Items; and *FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items; *FAR 252.246-7000, Material Inspection and Receiving Report. Also, Shaw 19 and 21 are applicable. Copies of Shaw clauses will be provided upon request. All contractors must be registered with the Central Contractors Registration to be eligible for award. Award will be made to the offeror, which offers the best value to the government based on an integrated assessment of price, delivery time, and suitability. All offers are due no later than 6 Aug 2004 at 4:00 p.m. EDT. *Complete Representations and Certifications are required with submission of pricing.
