- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W91364-04-Q-0043 is issued as a Request for SNAP-ON MECHANICS TOOL KIT P/N: KM180VMFCTK OR EQUAL. FOR COMPLETE LIST OF ITEMS IN KIT PLEASE CONTACT SRA ADAM POMORSKI AT adam.pomorski@ohtole.ang.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-24 and DCN 20040625. Items will be delivered to the 180th Fighter Wing Swanton, Ohio 43558. Solicitation provision at F AR 52.212-1, Instructions to offeror-commercial items (Jan 2004) is hereby incorporated by reference. FAR Clauses 52.212-4 Contract Terms and Conditions-Commercial Items (Oct 2003) and 52.212-5 Contract Terms and Conditions Required to Implement Statutes o r Executive Orders-Commercial Items (Jun 2004), are incorporated by reference. The following paragraphs apply to this solicitation and any resultant contract (b)(5)52.219-8, (b)(12)52.222-26, (b)(13)52.222-35, (b)(14)52.222-36, (b)(15)52.222-37, (b)(24)52. 232-33 and (b)(28)(I)52.247-64. DFARS Clause 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items, is hereby incorporated by reference, with additional DFAR cl auses included in this solicitation and any resultant contract, as follows: (b)(5)252.222-7001, (b)(15)252.225-7036. Additional Contract Terms and Conditions Applicable to this Procurement: Type of Contract-A firm fixed price contract will be awarded using the simplified acquisition procedures--multiple awards will be considered if more advantageous to the government. Award will be made to the offeror(s) that meets the solicitation criteria, and discussions- with business size and pricing being considered i n final award. The government reserves the right to eliminate from further consideration those proposals, which are considered to be non-responsive. Companies wishing to respond to the solicitation should provide the office with the following: A price quot e sent via fax (419-868-4222) - E-Mail (adam.pomorski@ohtole.ang.af.mil) - or US Postal Mail (180th Contracting Office, 2660 South Eber Road, Swanton OH 43558), Reference of the quotation number, TAX ID #, Cage Code, Prompt Payment Terms, Point of contact (Name/Phone/Fax/E-mail), and delivery time after receipt of order. Offers must be signed - those quotations received via e-mail will be considered signed. All questions regarding the RFQ can be addressed to SRA Adam Pomorski, 419-868-4314. All quotations a re due NO LATER THAN 12:00 PM EST 10 August 2004. Any quotation received after this time will be considered non-responsive. No oral quotes will be accepted. All responsible sources may submit a quotation, which shall be considered.
