- Description
- Contractor shall provide artificial limb fabrication, orthotic appliances and related services to eligible veterans of the VA Healthcare Network Upstate New York (Albany, Bath, Canandaigua, Syracuse, and Western New York) Medical Centers. Contractor performance shall include fabricating all artificial limbs in strict accordance to the prosthetic prescription provided by the VA Amputee Clinic Team and orthotic appliances in strict conformance to the Orthotic presrciption that has been submitted by the designated VA requesting provider. The VA shall require the Contractor to deliver items ordered under the contract no later than 30 calendar days after receipt of delivery order. The term of this contract will be one (1) year from date of award, with two(2) one year option renewals. The VA anticipates award of multiple contracts from this solicitation and will award a contract to responsible offerors whose offers are conforming to the solicitation and will be most advantageous to the Government, price and other factors considered. At least 80% of the contract awards will be set-aside for small business. The preferred method of payment will be Government Purchase/Visa Credit Card and the Contractor must be capable of accepting it for payment. The NAICS code for this procurement is 33911. This solicitation adheres to the format defined in FAR Part 12 for the acquisition of commercial items. The issue date for this solicitation will be on or about August 12, 2004.
