Description
The VA Pittsburgh Healthcare System (VAPHS) is seeking contractor(s) to provide primary care services to veteran beneficiaries at contractor's Community-Based Outpatient Clinics (CBOCs) located within Fayette and Washington Counties in Pennsylvania. The Fayette County CBOC is a new CBOC which was recently approved, and the enrollment at the end of the Base Year is estimated at 1,800. The Fayette County CBOC shall be located within the corporate city limits of Uniontown, PA, or no more than five (5) miles outside the corporate city limits of Uniontown, PA. The Washington County CBOC has been operated by a contractor since August, 2001, and the enrollment as of June 30, 2004, is 4,037. The Wshington County CBOC shall be located within the corporate city limits of Washington, PA, or no more than ten (10) miles outside the corporate city limits of Washington, PA. The Fayette and Washington County CBOCs shall be solely dedicated to veterans. The Government may make a single or multiple awards contingent upon best value and offeror's ability to meet all requirements. Oferors may submit a proposal on one or both sites. The conract is for a Base Year plus four (4) Option Year Periods. The solicitation allows a sixty (60) day start-up period. It is estimated that award will be made on or about November 4, 2004, and the first patient(s) will be seen on or about January 3, 2005. The estimated base year performance period is January 3, 2005 - January 2, 2006. The contractor shall be qualified to provide primary care services, such as, but not limited to: medical history, physcial exam, clinical assessment diagnosis, treatment, Level 1 laboratory services as specified in the solicitation), ECG's, x-rays (excluding invasive procedures, MRI's, CT and ultrasound), podiatry services, dietitian services, and mental health services provided by a licensed social worker (LSW). Physicians providing primary care services under this contract shall demonstrat evidence of education, training, and experience in Internal Medicine and/or Family Practice. The offeror will quote a "per member per month" capitation rate and an hourly rate for LSW mental health services for the Base Year and each option year and submit a written technical proposal addressing these evaluation factors: Past Performance, Staffing, Coordination and Continuity of Care, Site, and Small Disadvantaged Business (SDB) Participation Program. Hard copy and electronic versions of the solicitation will be available on or about August 9, 2004. Any amendments to the solicitation will also be available at the URL addresses, and offerors are advised that they are responsible for obtaining amendments. Requests for hard copy solicitation must be in writing to Janet Leuthold at the above address and accompanied by a non-refundable check for $5.00 made out to the VA Pittsburgh Healthcare System. No telephone requests will be accepted. A preproprosal conference will be held at the VAPHS, University Drive Division, Pittsburgh, PA; and the date, time, and conference room will be identified in the solicitation. Offerors may participate via conference call. The estimated proposal due date is September 13, 2004 (4:30 P.M.) EST. This solicitation is unrestricted.
