- The Performance Work Statement (PWS), dated June 30, 2004, paragraph 1.5.1. has been revised to state "Approximately 75 dental examinations are required to be performed by the Contract Health Care Provider on a typical UTA weekend". To receive a copy of the revised eight page PWS dated July 21, 2004 with Specific Job Duties and Employment Criteria, Pricing Schedule, and the Offeror Representations and Certifications contact Kendall Wagner, Contract Specialist, and provide vendor e-mail or fax information. Submit signed and dated quotations on company letterhead along with completed Pricing Schedule and Offeror Representations and Certifications to AAC/PKOBS, Attn: Kendall Wagner, 205 West D. Avenue, Suite 541, Eglin AFB, FL 32542-6864 at or before 4:00 P.M. Central Standard Time on 28 July 2004. Quotations may be faxed to 850-882-4916 or by E-mail at kendall.wagner@eglin.af.mil.
