Description
A Firm-Fixed Price Indefinite Delivery/Indefinite Order Quantity Requirements Contract to provide Home Oxygen and Durable Medical Equipment Maintenance to Department of Veterans Affairs, Veterans Integrated Network 18, for veterans beneficiaries in their homes. THIS SOLICITATION IS SUBJECT TO AVAILABILITY OF FISCAL YEAR 2005 FUNDS. HOWEVER, FUNDS ARE PROGRAMMED TO BE AVAILABLE FOR FISCAL YEAR 2005 CONTRACT PERFORMANCE. THIS IS A SOLICITATION ANNOUNCEMENT FOR REQUEST FOR PROPOSALS FROM QUALIFIED FIRMS INTERESTED IN PROVIDING THEIR OFFERS FOR HOME OXYGEN AND DURABLE MEDICAL EQUIPMENT MAINTENANCE. THE VETERANS INTEGRATED NETWORK 18 COMPRISES THREE STATES AND SEVEN VA FACILITIES. THERE ARE THREE FACILITIES IN TEXAS: EL PASO, BIG SPRING, AND AMARILLO. THERE IS ONE FACILITY IN NEW MEXICO: ALBUQUERQUE. THERE ARE THREE FACILITIES IN ARIZONA: PHOENIX, TUCSON AND PRESCOTT. The contractor shall provide and maintain home oxygen equipment for patients served by the VA facilities and their surrounding outpatient clinics. The main facilities are located at Amarillo, Big Spring, and El Paso, Texas; Albuquerque, New Mexico; Phoenix, Prescott, and Tucson, Arizona. Coverage will be 24 hours a day, seven days a week, including holidays. Work shall include, but is not limited to transporting, maintaining, cleaning, inspection, repair of home oxygen equipment as well as set-up and instruction in the use of such equipment to the patient and/or caregiver on the safe and appropriate use of equipment being supplied, including home safety, storage and handling of the equipment, with all required maintenance and electrical safety. The Contractor shall provide all service and equipment as specified by the Contracting Officer's Technical Representative nd according to the VA Home Oxygen Prescription Form, a copy of which will be provided to the Contractor. Contractor shall repair/clean, deliver/pickup, setup and store Government-owned and rented medical accessories (durable medical equipment) for VA Medical Centers or Clinics of Amarillo, Big Springs, El Paso, Texas; Albuquerque, New Mexico; and Phoenix, Prescott, and Tuscon, Arizona for beneficiaries in the clinic of jurisdiction. The Home Oxygen and Durable Maintenance Equipment Services will adhere to all VA Specifications, OSHA, and JCAHO Requirements. The following evaluation factors will be used to evaluate the interested firms using as a guide Federal Acquisition Regulation Part 52.212-2 Evaluation for Commercial Items: (1) Technical approach; (2) Past performance; (3) Price; (4) Safety Record. A competitive range will be established from the most highly rated proposals. The Government reserves the right to restrict the competitive range to the greatest number that will permit an effective competition among the most highly rated proposals. This will be established based on the number of responsible offers received based on the technical aspects of the solicitation and effective competition.
