- (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.603, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) Solicitation FA4877-04-Q0349 is being issued as a request for quote (RFQ). Submit written offers, oral offers will not be accepted. Please include specifications and pictures/graphs with your quotes.(III) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-22 (IV) This procurement is being issued as 100% SMALL BUSINESS SET-ASIDE. North American Industrial Classification Standard 332439, Size Standard NAICS), 500 employees, applies to this procurement. (V) Commercial Item Description: Qty- 8 each Fabricated Mobile Panel Storage Bins/Work Platforms, 2x2x1/8 sq.tube construction, .063 aluminum skin, rubber non-skid matting on top and on shelf surfaces. Removable handrails and ladders. 4 each for left side panels and 4 each for right side panels. Internal dimensions can be adjusted to maintain overall outside dimensions. Platform with heavy caster wheels should be capable of supporting 800 pounds (approximately 4 personnel and tools). Architectural license to change design to meet cost, safety, practicality, etc. granted with prior approval. Install lockable access doors on both sides of platforms 2 each Red, 2 each Blue, 2 each Yellow, and 2 each Black. Quote is to include shipping. (VII) FOB: Destination for delivery to 355 EMS, 3280 S. First Street, DAVIS MONTHAN AFB AZ 85707. (VIII) The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items. (IX) Provision 52.212-2, Evaluation- Commercial Item Applies. Quotes will be evaluated on price related factors only. (X) The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, and the offeror must include a completed copy of this provision with their proposal. (XI) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items and any addenda the clause applies to this acquisition. (XII) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items applies to the award document.
