Description
PRESOLICITATION NOTICE FOR COMMERCIAL PHARMACY SERVICES AT THE ARMED FORCES RETIREMENT HOME IN GULFPORT, MS. BACKGROUND: The United States Congress established the Armed Forces Retirement Home (AFRH) as a separate Federal Agency in 1991 when it enacted the Defense Authorization Act, Public Law 101-510. The AFRH is comprised of the Armed Forces Retirement Home in Washington, D.C. (AFRH-W), and the Armed Forces Retirement Home in Gulfport, Mississippi (AFRH-G). Each site offers independent, Long-Term Care/Skilled Care, Assisted Living and Special Care (dementia) options for its residents. Each site is maintained as a separate establishment of the AFRH for administrative purposes. This contract applies only to AFRH-G. The mission of the AFRH is to provide residents with a model retirement center, where residents can maintain an independent lifestyle in an environment designed for safety, comfort, and personal enrichment. The total number of residents on the rolls of the AFRH-G will vary from month to month, but not widely, as influenced by admissions, deaths, and separations. The actual number to be medical care recipients will also be influenced by the number who are on leave or who may be patients at an outside medical facility. AFRH-G currently has 590 residents but can support up to 620. Medicare Part A does not cover all of these residents nor does AFRH-G receive reimbursement from Medicare Part A. Residents? physical and mental conditions and capabilities vary widely. SCOPE: The Contractor shall provide pharmacy services to the AFRH-G including but not limited to accurate and reliable dispensing of medications specifically using ?blister packs?, providing drug regimen reviews, medication pass reviews, in-service staff training, maintenance of applicable records and providing all related pharmacy services in accordance with all professional and licensure codes. The Contractor shall participate in chart reviews monthly, assist in the Performance Improvement Program including attending Performance Improvement meetings every 4-6 weeks, and assist in poly-pharmacy and psychotropic reviews and recommendations and participate in monthly inpatient unit inspections. The Contractor shall meet the needs of the Long-Term Care/Skilled Care (LTCU), Assisted Living (ALU) and Special Care (dementia) Unit residents by providing accurate and dependable dispensing of medications. Current average age of the population is 78. The average number of medications per individual is ten (10). The Contractor shall provide personnel (pharmacists, pharmacy technicians and nurses or other personnel) having proper credentials and licensed in accordance with federal and state regulations as required for the tasks being performed. The Contractor shall furnish managerial, administrative, and direct labor personnel to accomplish all work specified in the Statement of Work (SOW). The workforce shall be adequately supervised and trained at all times. The contractor shall supply all materials, supplies, and equipment not expressly stated in the SOW. The AFRH-G will provide existing pharmacy space, computer, utilities and appropriate furniture and fixtures. Government employees will jointly use the pharmacy space and associated equipment. The AFRH-G will provide storage and control of medications comprising the ?Documed?. HOURS OF OPERATION: The Contractor shall provide medications on a daily basis, five (5) days a week normally from 8:00 AM through 5:00 PM Monday through Friday. After Hour Services: The Contractor shall have a local back-up pharmacy, and a consultant pharmacist on-call for emergency delivery of medications 24 hours a day, seven days a week including holidays. Special Requirements: During reviews, surveys and inspections by external groups or organizations (JCAHO, etc.), the Contractor shall accommodate the reviewers' schedules at the site or via telephone conference. TASKS AND SERVICES 8.1 Prescription Services 8.1.1 The Contractor shall provide medications in unit dose ?blister packs?, or individual liquid dispensing containers or tubes. 8.1.1.1 ?Blister Pack? is defined as packaging of medication, in pill form, contained on a punch card of up to one full month?s supply. The Contractor will blister pack VA or Keesler bulk medications for residents who have no insurance. 8.1.1.2 Individual liquid dispensing containers and tubes are defined as bottles or jars and squeeze tubes containing a volume of medication designed for individual consumption or use. 8.1.2 All prescriptions including ?Blister packs? shall, at a minimum, be labeled with the following information: The name of the drug and its dosage. The recipient and prescribing physician?s name. The recipient?s Social Security Number. The number of authorized refills. The expiration date. All safety and consumption warnings. The date ordered. 8.1.3 The Contractor shall use generic medications whenever possible. 8.1.4 The Contractor shall meet the following response time standards: 8.1.4.1 Routine Prescriptions The Contractor shall provide routine prescribed medication in blister packs or individual prescribed liquid medications such as antacids, Metamucil, etc., and ointments as soon as practical but not later than the next working day. 8.1.4.2 Emergency Prescriptions The Contractor shall provide emergency prescribed medication within eight (8) hours including holidays and weekends. 8.2 Required Meetings and Consulting Services 8.2.1 Drug Regimen Reviews The Contractor shall perform and have the expertise in doing drug regimen reviews and offering therapeutic substitutions and recommendations. 8.2.2 In-Service Staff Training The Contractor shall provide in-service staff training on medications. 8.2.3 Drug Reviews and Medication Pass Reviews The Contractor shall perform periodic drug reviews and medication pass reviews. Drug reviews and medication pass reviews shall be conducted monthly at the beginning of contract performance and shall meet JCAHO compliance requirements. 8.2.4 Performance Improvement Program The Contractor shall attend Performance Improvement Program meetings at intervals required by AFRH-G (generally every 4 - 6 weeks). The Contractor shall assist in the Performance Improvement Program, including but not limited to, recommendations to healthcare providers on decreasing poly-pharmacy, psychotropics, and evaluations and reviews as appropriate within the dictates of JCAHO, AFRH-G policies and procedures, and other regulatory agencies. 8.2.5 Interdisciplinary Team Conferences Interdisciplinary Team conferences are conducted at AFRH-G weekly at the Long-Term Care/Skilled Care (LTCU), Assisted Living (ALU) and Special Care (dementia) Units. The Contractor shall ensure its recommendations and any pertinent information regarding medications are provided for these conferences. The Contractor shall provide pertinent information for quality of care concerning pharmacology during monthly chart reviews. The Contractor shall attend conferences and reviews as required/requested. 8.2.6 Other Briefings, Meetings and Conferences The Contractor shall attend other meetings, conferences, and briefings as required. BILLING/PAYMENT: AFRH-G residents have Medicare, TRICARE or CHAMPUS and some have a supplementary medical insurance policy. At the present time, TRICARE requires that services be requested through the Primary Care Physician in order for TRICARE to be billed for services. Insurance information and Informed Consent will be provided to the Contractor by the residents of the AFRH-G at the time services are rendered. The Contractor shall ensure the residents fully understand that their private insurance will be billed for services. In the case of a resident who is under a conservatorship, the AFRH-G will be responsible for providing insurance information. AFRH-G will also assist with Informed Consent if applicable, related to the necessity of all ?inpatient residents? receiving medication administration. Third Party Billing - The Contractor shall bill residents? insurance companies directly for reimbursement of pharmacy prescription services. Residents and/or their insurance companies shall be responsible for payment of services including co-payments. The Government is NOT responsible for payment, including co-payments, and the Contractor shall not bill the Government for these services. SITE VISIT: A site visit is scheduled at AFRH-G in Gulfport, MS at 1:00p.m. CST on Friday, July 9, 2004. You must contact the contracting officer via email NLT noon on July 8, 2004 with your company name and attendees to ensure entrance onto the facility through security.
