- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W911S8-04-Q-0045 is applicable and is issued as a Request for Quotations. The solicitation and incorporated provisions and clauses are those in effect from Federal Acquisitio n Circular 2001-15. The requirement is a small-business set-aside with a NAICS code of 423690, and a size standard of 100 employees. The description of the commercial item is as follows: CLIN 0001: 1(one) each - Actall Corporation PALS 9000 personal duress and locator system, to include delivery and installation of the system. The system shall include a minimum of 35 each - interior locators, 12 each - exterior locators, 2 each - repeaters, 1 each - workstation, 1 each - software package, 1 each - receiver assembly, 60 each - personal duress alarm transmitters, and all required power supplies, connectors, boxes, wiring, conduit, etc., necessary to set up the system. The system shall be installed at the Regional Correctional Facility at Fort Lewis, WA. FOB sh all be Destination. State delivery lead-time. Any award resulting from this Request for Quotations shall be made to a responsive, responsible quoter using an SF1449 form. Quotes will be evaluated on a lowest price evaluation factor. Provision 52.212-1, Ins tructions to Offerors-Commercial Items, applies to this acquisition. An addendum to this provision is applicable in which the term offeror, wherever it appears, shall be replaced by the term quoter. Quoters are required to submit completed copies of the pr ovisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, and DFAR 252.212-7000, Offerer Representations and Certifications-Commercial Items, with their quotes. FAR clause 52.212-4, Contract Terms and Conditions, applies to th is acquisition and does not include any addenda. FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. The following clauses within 52.212-5(b) are applicable to this acquisition: 52.203-6, 52.219-4, 52.219-8, 52.219-14, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, and 52.232-33.The following clauses within DFARS clause 252.212-7001(b) are applicable to this acquisition: 5 2.203-3, 252.225-7001, 252.225-7021, 252.243-7002, and 252.247-7023 are also applicable..
