//THE CLASSIFICATION CODE IS HEREBY AMENDED TO READ "53".// This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format listed in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The solicitation number is FA4621-04-Q-0045. This solicitation is a request for quotation (RFQ). The Solicitation Document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Fac 2001-22 & 23, Effective 05 May 2004 and Fac 2001-19, Effective 8 Jun 2004 & Class Deviation 2004-o0001. The solicitation is 100% Small Business set aside. The NAICS/SIC code is 444130/5251 and the small business size standard is “500 employees.” This solicitation is for the purchase of the following: CLIN 0001-CYLINDER DEADLOCK, DL3000 SERIES, SINGLE CYLINDER, OPERATES BY KEY ON OUTSIDE AND BY THUMB TURN ON INSIDE, 2-3/4" BACKSET, 1” DEADBOLT THROW, US26D SATIN CHROME PLATED, INTERCHANGEABLE CORE (IC), 6 PIN, 0-BITTED WITH 4 BLANK KEYS/UNIT, CORBIN KEYWAY 59A1. MFG NAME: CORBIN/RUSSWIN MFG PART NO. DL3013-626-C6-KY4-59A1 (QUANTITY 198 EA). No substitutions allowed as solicited items must conform to MAB master keying/security programs. All items shipped FOB Destination to McConnell AFB, KS 67221. Submission of offers: Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted by letter, FAX or email. Offers must show (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, email address and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation; (5) Terms of any express warranty; (6) Price and any discount terms; (7) A completed copy of the representations and certifications at FAR 52.212-3; (8) Acknowledgment of Solicitation Amendments, if applicable; (9) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information), if applicable; (11) Contractors submitting quotes must be enrolled in the Central Contractor Registration Program (CCR); (12) Include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Clauses Incorporated By Full Text: 52.212-2 Evaluation--Commercial Items (Jan 1999) (a)The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Awards shall be made to the offeror who provides the overall best value to the government based on price. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.
