Description
Under Statutory Authority 41 U.S.C 253 (1) the Government anticipates entering into sole source procurement negotiations for Brand Name Only Medical Equipment under this combined synopsis and procurement as Request for Quotation RFQ 618-193-04 entitled Brand Name Only Upgrade of hardware/software to Ensite NavX Version 4.2 to VAMC’s existing EnSite 3000 SYSTEM with Endocardial Solutions, St. Paul, MN; this is an upgrade to arrythmia monitoring equipment for Cardiology Department, VA Medical Center, Minneapolis, MN. This is a combined synopsis/solicitation issued for commercial items and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures. This announcement constitutes the only solicitation; a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-23. NAICS code 423430, size standard of 100 employees applies; seeking (1) Brand Name only EnSite NavX v.42 Upgrade model CE0900 with one-year extended warranty, including 24 hour response time, 48 hour system repair time, 72 hour loaner exchange time covering Computer Processing Unit (CPU), monitor(s), keyboard, Patient Interface Unit (PIU), breakout box components, breakout box cable, fiberoptic Ethernet cable, PIU cable, and Hardware consisting of CPU upgrades (Intel Xeon Workstation & DVD with NVIDIA Quadro 4 graphics technology), breakout box upgrades (version 4.2), PIU upgrades, and Software maintenance & support (version 4.X), software revisions (version 4.X), and new software releases (beyond version 4.X) at no additional charge for a 12-month period to ensure availability of most current mapping and navigation technology. Vendor will provide on-site assistance and training. Vendor proposals are hereby being requested in response to requirements listed and shall be submitted on vendor’s letterhead and must address all requirements listed. Vendor response should contain the name, title and signature of person authorized to submit proposals/bids on behalf of the contractor. Delivery shall be FOB Destination to VA Medical Center, Warehouse (90D), One Veterans Drive, Minneapolis, MN 55417. Evaluation and/or award may be made without discussions. All responsible sources that can meet the requirements and provide items as listed above may respond to this solicitation by submitting cost and technical proposals on their company letterhead.
