Description
NA A/E DESIGN SERVICES – SOURCES SOUGHT: Provide professional A/E DESIGN services to include site evaluation, Determination of Requirements, recommendations, construction phasing recommendations, planning, consultation and visits, design work, preparation of contract drawings and specification, preparation of contract drawings and specifications, preparation of cost estimates and construction period services as necessary for demolition construction, inspection, testing and certification of all work and design for the “Inpatient Privacy - 4B North" PROJECT NUMBER 534-315 at the Veterans Affairs Medical Center, Charleston, SC. Specifically this project will renovate approximately 9,850 SQ. FT. of inpatient medicine ward 4BN to create new private/semi-private toilet/shower facilities in existing patient rooms. Three existing congregate bath/shower rooms serve both 4BN and 4BS patients providing little or no patient privacy. Existing space will be redistributed by removing space from older over-sized multi patient rooms and congregate facilities and providing that space to older under-size private rooms, storage, clinical support services, and private/semi-private toilets/shower. New spinal cord injury and tuberculosis isolation rooms will be included near nurse’s station. Temporary insulation rooms will be necessary outside 4BN construction site to replace existing isolation rooms removed form service during 4BN construction. The projected number of patient beds is 20. All professional services shall be part of the design team announced in the 254s and 255s. Final plans and specification are required for issue by June 1, 2005. EVALUTION FACTORS, NOT IN PRIORITY ORDER SHALL BE: 1) TEAM PROPOSED FOR THIS PROJECT; 2) PREVIOUS EXPERIENCE OF PROPOSED TEAM; 3) PROPOSED DESIGN APPROACH FOR THIS PROJECT AND MANAGEMENT PLAN; 4) Proposed Management Plan: Team organization for conceptual, design and construction phase. Ability to meet project specific deadlines. 5) LOCATION AND FACILITIES OF WORKING OFFICES WITHIN A 250-MILE RADIUS OF CHARLESTON, SC, INCLUDING THE PRIME FIRM AND CONSULTANTS; 6) CONTROL OF CHEDULE/COST/ESTIMATING EFFECTIVENESS OF 10 MOST RECENT PROJECT, AUTOCAD 2002 CAPABILITIES, INTERNET ACCESS, PREVIOUS LITIGATION INVOLVEMENT, ETC. A/E firms are required to respond in writing if interested and shall submit a completed SF 254 (A/E and Related Services Questionnaire) and SF 255 (A/E Related Services for Specific Projects) to the following: Chief, Network Logistics Office (544/10N7/NLO), Attn: Tresa Goodwin, Contract Specialist and Diana Nance, Contracting Officer, VA Medical no later than July 9, 2004. Estimated construction cost range: $2M - $5M. This SOURCES SOUGHT is restricted to 8(a). Applicable NAICS is 541310 with small business size of $4 Million. VA Point of Contact: William M.
